Background. Exercise equipment such as treadmills are becoming commonplace in residential homes, placing small children at risk for injury. These injuries can be severe and may require surgical intervention. While it is our clinical perception that these injuries are on the rise, they remain largely unreported in the literature. Therefore, we reviewed our experience to evaluate the incidence and outcomes of treadmill-associated injuries in children. Methods. After receiving exempt IRB approval, we retrospectively reviewed all patients who sustained treadmill-related injuries that required evaluation by a surgeon from July 2005 to February 2010. Data collected included patient demographics, injury details, injury management, and outcomes. Results. We identified 19 children who required treatment for treadmill-related injuries. Mean age at injury was 4.1 y (1.3-10.5 y), and 63% were male. The treadmill was in use by another person in 17 cases (89%). The hand was involved in 79%. All burns were < 10% body surface area and 18 (90%) were < 5 %. Admission was required in two cases, and four (21%) children required skin grafting. Healing was complicated by hypertrophic scarring in four patients (21%). Mean length of active therapy was 9.2 +/- 7.0 d and involved a mean of 6.0 +/- 3.5 healthcare visits. Mean hospital charges were $5700. Conclusion. Treadmill-related burn injuries in children are preventable injuries that can pose a substantial burden on patients and families. Supervision is paramount in prevention of these injuries, and strategies should include child safety features in equipment designs along with consumer awareness. (C) 2011 Elsevier Inc. Asymptomatic non-occult pneumothorax in pediatric blunt chest trauma: Chest tube versus observation

Obiyo Osuchukwu, Joseph Lopez, Katrina L. Weaver, Valerie A. Waddell, Pablo Aguayo, Shawn D. St Peter, David Juang

Summary: In pediatric patients, CT is not necessary for all asymptomatic non-occult pneumothoraces (ANOPTX) from blunt chest trauma (BCT), and observation has been found to be safe and cost-effective. Patients who underwent CT placement had longer hospital stays, higher hospital expenses, and more imaging performed compared to those who were observed.

Shai Stewart, James A. Fraser, Rebecca M. Rentea, Pablo Aguayo, David Juang, Jason D. Fraser, Charles L. Snyder, Richard J. Hendrickson, Tolulope A. Oyetunji, Shawn D. St. Peter

Summary: This study analyzed the outcomes of pediatric patients with primary spontaneous pneumothorax (PSP) who were managed with a protocol involving simple aspiration and, if necessary, Video-Assisted Thoracoscopic Surgery (VATS). The results showed that 33% of patients were successfully treated with aspiration alone, while 66% required VATS. Simple aspiration is a safe and effective initial management for children with PSP, but VATS may be necessary in most cases.

Summary: Introduction: Prescription opioids, including those prescribed after surgery, have greatly contributed to the US opioid epidemic. Educating opioid prescribers is a crucial component of ensuring the safe use of opioids among surgical patients. This study implemented an annual education curriculum for new surgical prescribers, resulting in significant improvements in knowledge and comfort levels. However, there remains a persistent knowledge and comfort gap among these prescribers.

Humanized Anti-Carcinoembryonic Antigen Antibodies Brightly Target and Label Gastric Cancer in Orthotopic Mouse Models

Summary: This study demonstrates the potential of using humanized anti-carcinoembryonic antigen (CEA) antibodies conjugated with near-infrared dyes to specifically label gastric cancers in mouse models. Orthotopic models showed bright and specific labeling with more than ten times higher tumor-to-background ratios compared to the control. This tumor-specific fluorescent antibody has promising potential as a clinical tool for improving visualization of gastric cancer margins during surgical resection.

Use of Broadband Personality Tests in Bariatric Surgery: A Scoping Review

Sarah Maki, Melissa Leon, Emily Glenn, Tiffany Tanner, Crystal Krause

Summary: This scoping review analyzed the literature on the use of broadband personality tests in the bariatric surgical population to optimize weight loss outcomes. The study found significant associations between personality scales and weight loss, but inconsistent reporting of outcome measures made it challenging to draw concrete conclusions. The American Society for Metabolic and Bariatric Surgery recommends standardization of outcome reporting to improve the reliability of predicting weight loss outcomes.
