Summary: This study aimed to analyze the occurrence of wound complications (WC) and identify risk factors in patients with soft tissue sarcoma who underwent preoperative radiotherapy followed by surgical resection. The incidence of WC and major wound complications (MaWC) was found to be 43.7% and 19%, respectively. Age, tumor size, and tumor site were identified as risk factors for WC, while smoking and the time interval between surgery and wound complication were risk factors for MaWC. The MaWC rate was comparable to that of postoperative radiotherapy and surgery alone.

Gross macroscopic incomplete resection of first, second, and third recurrences is an independent poor prognostic factor for OS. The results showed that rPF was significantly higher in cases of local recurrence, while rMTT was slightly lower. rPF demonstrated higher accuracy and specificity, allowing for differentiation between local recurrence and benign postoperative changes.

Summary: Soft tissue sarcomas, originating from mesenchymal tissue in the extremities, are commonly treated with surgery and radiotherapy. This study found that patients with lower lymph-sparing volume in adjuvant radiation therapy for extremity soft tissue sarcoma have an increased risk of lymphedema. Maximizing the potential oncologically-justifiable lymph-sparing volume should be considered to reduce the risk of high-grade lymphedema in extremity radiation therapy.
