Incidence Rate of Cerebrovascular Diseases in Northern Japan Determined from the Iwate Stroke Registry with an Inventory Survey System

Background: Long-term stroke registries in large populations often fail to maintain accuracy. This study presents an inventory survey system for stroke registries and the incidence rate of cerebrovascular diseases in a region with the highest stroke mortality rate in Japan. Methods: Using the data of the stroke registry determined with the inventory survey from 2004 to 2008 in northern Iwate Prefecture, with a population of 235,280 (111,584 men and 123,696 women), the age-specific annual incidence rates, the age-adjusted annual incidence rates, and the ratio of incidence rate relative to mortality rate of the cerebrovascular diseases were investigated. Results: A total of 3415 cases (1714 men and 1701 women) were registered and analyzed in this study. The age-adjusted incidence rates by the 1985 model population of Japan and by the world standard population (range 35-64 years) were 100.4 and 89.4 per 100,000 population in men and 49.8 and 29.7 in women for cerebral infarction; 53.6 and 77.2 in men and 34.2 and 39.5 in women for intracerebral hemorrhage; 12.9 and 23.3 in men and 21.1 and 34.6 in women for subarachnoid hemorrhage; and 166.9 and 189.8 in men and 105.0 and 103.7 in women for all subtypes. The ratios of incidence rate relative to mortality rate were 1.66 for all stroke subtypes, 1.69 for cerebral infarction, 1.76 for intracerebral hemorrhage, and 1.31 for subarachnoid hemorrhage. Conclusions: High incidence rates of cerebrovascular diseases were revealed in the stroke registry with a good inventory survey in northern Iwate Prefecture, Japan.

Risk Factors of Long-Term Care Insurance Certification in Japan: A Scoping Review

Summary: This study aimed to review evidence on the association between pre-existing factors and future long-term care among community-dwelling Japanese older adults. Common risk factors identified included physical function, frailty, oral condition for functional disability, and motor function, nutritional status for dementia. Consistent associations were found between premorbid risk factors and functional disability and dementia, highlighting the importance of preventive measures in long-term care planning.

Human Plasma Xanthine Oxidoreductase Activity in Cardiovascular Disease: Evidence from a Population-Based Study

Summary: Xanthine oxidoreductase (XOR) and its products are important contributors to chronic inflammation and oxidative stress. Excessive XOR activity is believed to promote inflammatory responses and the formation of atherosclerotic plaques, which are major cardiovascular risk factors. The mechanisms of XOR activity in the development and progression of cardiovascular disease (CVD), as well as the complex relationship between XOR activity and the biological effects of uric acid, reactive oxygen species, and nitric oxide, have been debated but not yet clearly elucidated. Recent advancements in measuring XOR activity in human plasma and accumulating findings on the relationship between XOR activity and CVD have shed light on this topic. In this narrative review, we summarize the current knowledge on plasma XOR activity and its association with CVD, and discuss future perspectives.

Rapid weight change as a predictor of disability among community-dwelling Japanese older adults

The associations between weight change and disability risk are more pronounced in the unhealthy lifestyle and lower initial normal body mass index groups.

Safety and efficacy of endovascular thrombectomy for primary and secondary MeVO

Summary: This study aimed to evaluate the efficacy and safety of endovascular treatment (EVT) in primary and EVT-induced secondary medium-vessel occlusions (MeVO). The results showed that primary MeVO had poorer outcomes in terms of complications, EVT efficacy, and the proportion of patients with a modified Rankin Scale (mRS) of 0-2 at 3 months compared to exclusive large-vessel occlusion (LVO). However, EVT-induced secondary MeVO showed similar safety and efficacy outcomes as exclusive LVO and had better outcomes in terms of mRS 0-2 compared to primary MeVO.

Summary: Compared with clopidogrel, DAPT with ticagrelor plus aspirin was associated with significantly lower thromboembolic events without an increase in hemorrhagic events in the treatment of intracranial aneurysms, according to the one-year clinical outcome analysis. However, larger upcoming trials are needed to confirm the effectiveness and safety of ticagrelor-associated DAPT.

Summary: This study assessed the geographic accessibility of stroke rehabilitation facilities in Tennessee. The study found that rural counties had significantly longer travel distances and times compared to urban counties. Counties with high and medium stroke prevalence also had higher travel times and distances compared to low stroke prevalence counties. Therefore, addressing transportation barriers and implementing tele-rehabilitation are crucial for improving access to stroke aftercare in these areas.

Role of serum alkaline phosphatase as a potential biomarker of severity and prognosis in intracerebral hemorrhage

Summary: The study aimed to investigate the relationship between serum alkaline phosphatase (ALP) levels and the severity and prognosis of intracerebral hemorrhage (ICH). The results showed that serum ALP levels in ICH patients were significantly elevated and correlated with hematoma volume and Glasgow coma scale (GCS) score. High serum ALP levels may serve as a potential predictor for the severity and unfavorable prognosis of ICH.
