Construct Validity and Reliability of the Disability of Arm, Shoulder and Hand Questionnaire for Upper Extremity Complaints in Rheumatoid Arthritis

Objective. The Disability of Ann, Shoulder and Hand (DASH) questionnaire is a tool for measuring physical function and symptoms of the upper extremity. Although widely used, it is not validated for rheumatoid arthritis (RA). In this study the DASH was validated for this patient group. Methods. In total, 102 patients participated in this study. For the validation, the questionnaires of the DASH, the Health Assessment Questionnaire (HAQ), the Medical Outcomes Study Short Form-36 (SF-36), and the Arthritis Impact Measurement Scale (ATMS2) were used. Patients were examined clinically before completing the questionnaires. Pain was scored by each patient using a visual analog scale (VAS). The Disease Activity Score (DAS28) was obtained and grip strength was measured. Reliability was tested by a second DASH questionnaire after 2 days. Validity was tested using a Pearson correlation analysis for the relevant domains of the questionnaires and for the clinical aspects. Results. The reliability of the DASH was excellent (intraclass correlation coefficient 0.97). Internal consistency was strong (Cronbach's alpha 0.97). Validity was proven with excellent results for Pearson correlation with the relevant domains of the questionnaires: HAQ, r = 0.88; SF-36, r = 0.70; and AIMS2, r = 0.85. The clinical scores had a relatively low correlation with the DASH (DAS28, r = 0.42; and grip strength, r = 0.41-0.48), except for the VAS (r = 0.60-0.65). Conclusion. The DASH is a reliable and valid questionnaire in patients with RA. It can be used as a measurement tool of physical disability of the upper extremity. Summary: This study compared the migration characteristics of hydroxyapatite coated (HA+) and uncoated (HA-) Zweymuller-type hip stems through a double-blinded randomised RSA trial with a 5-year follow-up. The results showed that the addition of a hydroxyapatite coating did not influence the migration of the hip stem after 5 years, and there were no significant differences in clinical outcomes between the two groups.

Summary: This study analyzed data from the Dutch Arthroplasty Register and found that ceramic-on-ceramic (CoC) bearings had a higher 2-year cup revision rate compared to ceramic-on-polyethylene (CoPE) bearings in press-fit total hip arthroplasty. The main reason for revision in CoC bearings was cup loosening.

Limited knee extension during gait after total knee arthroplasty is related to a low Oxford Knee Score

Summary: There was a significant relationship between the difference in knee extension angle during gait between patients and controls with the Oxford Knee Score (OKS), where an increase of 1 degree in difference led to a reduction of 1.0 to 1.6 points in OKS. Patients overall exhibited less knee extension compared to controls, while ankle and hip gait parameters and joint moments did not show any correlation with OKS.

Summary: This study aimed to investigate whether consulting an occupational medicine specialist (OMS) within 3 months after total knee arthroplasty (TKA) surgery can lead to an earlier return to work (RTW) for patients. The results showed that consulting an OMS did not result in a decrease in time to RTW for TKA patients. However, TKA patients with high expectations returned to work earlier without consulting an OMS.

Summary: The purpose of this study was to assess the failure rate, revision rate, and functional outcomes of treating acute ACL rupture with the Ligamys device. It was found that the failure rate of the device was 30.2%, which did not meet the non-inferiority criterion compared to ACL reconstruction. However, good functional outcomes were observed in patients without the need for secondary reconstructive surgery, even in the presence of anteroposterior knee laxity.
