Summary: Previous research has shown that online resources about breast reconstruction surgery have low readability, but specific online resources about autologous breast reconstruction have not been evaluated. This study assessed the readability of online patient-directed resources about the DIEP and TRAM flaps, which are commonly used in breast reconstruction. The results indicated that the readability level of both DIEP and TRAM resources were above the recommended 6th-grade reading level, and there was no significant difference in readability between the two procedures.

Summary: This study investigated the overall outcomes of deep inferior epigastric perforator (DIEP) flap reconstructions in Germany using the breast reconstruction database established by the German Society of Plastic, Reconstructive and Aesthetic Surgeons (DGPRaC). The study included data from 3926 patients and 4577 DIEP flaps performed by 22 centers, showing low rates of total and partial flap loss, emergency vascular revision surgery, and revision surgery due to wound complications. Higher volume centers had significantly shorter operative time and length of hospital stay. Overall, the outcome and complication rates highlighted the high standard of microsurgical breast reconstruction in Germany at a national level.

Summary: This study compared the treatments and time intervals between treatments of breast cancer patients diagnosed during and before the COVID-19 pandemic, and the number of treatments started during and before the pandemic. The results showed that patients diagnosed during the transition period were less likely to receive neo-adjuvant chemotherapy for stage I, stage II, and hormone receptor+/HER2- tumors compared to 2018/2019. The time between diagnosis and first treatment decreased for patients diagnosed during the lockdown period. Additionally, there was an increase in the number of neo-adjuvant endocrine therapies and surgeries starting in week 14 of 2020, while the number of adjuvant chemotherapies decreased in week 15 and increased in week 22.

Summary: This article introduces the Areola study, which aims to assess the risks of breast implant illness (BII) and autoimmune diseases in Dutch breast cancer survivors with silicone breast implants. The study design includes a retrospective cohort of breast cancer survivors who underwent implant-based reconstruction surgery between 2000 and 2015 in six major hospitals in the Netherlands. A comparison group of breast cancer survivors without breast implants and a group of women who received breast augmentation surgery will also be included. The study will collect data through a web-based questionnaire and link it with population-based databases to evaluate the prevalence, incidence, and risk factors of BII and autoimmune disorders.

Summary: This study investigated the techniques and opinions of Dutch plastic surgeons and oncological breast surgeons regarding the preservation of pectoral fascia (PF) during mastectomy. The results showed that approximately one in five plastic surgeons and breast surgeons routinely preserve the PF, and even more surgeons preserve the PF in specific cases. There were significant differences in surgical techniques and opinions regarding PF preservation among the surgeons. Future studies should clarify the impact of PF preservation on oncological safety, complication rates, postoperative pain, cosmetic outcomes, and patient satisfaction.

The main endpoint of the study is the difference in clinically relevant fat necrosis between the two groups evaluated at two weeks and three months after reconstruction.

Summary: The current literature on reconstruction after head and neck cancer mainly focuses on short-term patient-reported outcomes (PROs), but more knowledge is needed on long-term consequences. A systematic review of 30 studies with 2358 patients revealed that reconstructive surgery generally leads to diminished oral function, worsened by radiotherapy. Patients experience anxiety and fear of cancer recurrence, but there is a progressive decrease in pain over time with some flaps having more favorable health-related quality of life (HRQoL) outcomes. Age and bony tumor involvement are not associated with postoperative HRQoL.

Except for axillary lymph node dissection (ALND) with axillary radiotherapy, other axillary treatments showed no significant impact on physical and psychosocial wellbeing. Patients who received ALND with radiotherapy experienced a significant decrease in physical and psychosocial wellbeing postoperatively.

Summary: This study analyzed complications following cosmetic surgeries managed by plastic surgery services at Heatherwood and Wexham Park hospitals in 2022, including procedures performed internationally. The majority of complications were related to breast cosmetic surgery and abdominoplasties, with the most common complications being wound dehiscence, post-operative infection, and seromas. Turkey was the most popular country selected for surgery, and there is a need to increase public awareness of the risks involved in seeking international options and how to choose suitable clinics.

Summary: The study aims to compare the oncologic, medical, and surgical outcomes of lumpectomy and oncoplastic breast reduction surgery (OBRS) on a national scale. A database was used to analyze patient data and match them based on various factors. The results showed that lumpectomy patients had higher rates of repeat lumpectomy or subsequent mastectomy, while OBRS patients had higher rates of surgical complications but fewer medical complications. Logistic regression analysis revealed that OBRS was associated with a decreased likelihood of repeat lumpectomy. Although OBRS was associated with increased wound complications, medical complications were found to occur less frequently.
