Facial paralysis refers to a condition in which all or portions of the facial nerve are paralysed. The facial nerve controls the muscles of facial expression, paralysis which results in a lack of facial expression which is not only an aesthetic issue, but has functional consequences as the patient cannot communicate effectively. The treatment of long-standing facial paralysis has challenged plastic surgeons for centuries, and still the ultimate goal of normality of the paralysed hemi-face with symmetry at rest as well as the generation of a spontaneous symmetrical smile with corneal protection has not yet fully been reached. Until the end of the 19th century, the treatment of this condition involved non-surgical means such as ointments, medicines and electrotherapy. With the advent and refinement of microvascular surgical techniques in the latter half of the 20th century, vascularised free muscle transfers coupled with cross-facial nerve grafts were introduced, allowing the possibility of spontaneous emotion being restored to the paralysed face became reality. The aim of this article is to revisit the surgical evolution and current options available as well as outcomes for patients suffering from facial paralysis concentrating on middle and lower face reanimation. Summary: This study aimed to identify individual risk factors leading to flap failure and vascular compromise in free tissue transfers and to create a predictability index. Patients with moderate and high-risk indices had significantly higher chances of vascular compromise. Additionally, the American Society of Anesthesiologists (ASA) classification was found to be a predictor of complications in free tissue transfer.

Summary: The study found that reinnervation occurs after nerve repair or graft, but the number and calibre of regenerating axons can vary, leading to smaller motor units. While all NMJ were reoccupied by regenerating axons, there were significant differences in the way small axons are remodelled between nerve repair and nerve graft groups.

Atypical Pleomorphic Lipomatous Tumor of the Right Hand Mimicing Venous Malformation

Summary: The study found that mentioning medical indications in the application letter increased the chances of insurance approval, while mentioning mental suffering decreased the likelihood of a positive response. High initial BMI and BMI reduction were associated with higher approval rates by insurance companies. Submitting an additional application letter to the insurance company and mentioning mechanical restrictions were also found to be associated with positive responses.

Summary: This study analyzed complications following cosmetic surgeries managed by plastic surgery services at Heatherwood and Wexham Park hospitals in 2022, including procedures performed internationally. Turkey was the most popular country selected for surgery, and there is a need to increase public awareness of the risks involved in seeking international options and how to choose suitable clinics.

Oncoplastic breast reduction surgery decreases rates of reoperation with no increased medical risk

Summary: The study aims to compare the oncologic, medical, and surgical outcomes of lumpectomy and oncoplastic breast reduction surgery (OBRS) on a national scale. A database was used to analyze patient data and match them based on various factors. The results showed that lumpectomy patients had higher rates of repeat lumpectomy or subsequent mastectomy, while OBRS patients had higher rates of surgical complications but fewer medical complications. Logistic regression analysis revealed that OBRS was associated with a decreased likelihood of repeat lumpectomy. Although OBRS was associated with increased wound complications, medical complications were found to occur less frequently.
