Summary: Basal cell carcinoma (BCC) is the most common skin cancer, but metastasizing BCC (MBCC) is rare, occurring in approximately 0.0028% to 0.55% of BCC patients. We report two cases of pulmonary MBCC. The first case is a 72-year-old male with multiple recurrences who underwent surgical resection and adjuvant radiotherapy. Immunohistochemistry showed the expression of Ber-EP4, CK5/6, p63, EMA (focal), BCL-2, and CD10 in neoplastic cells, while they were negative for CK7, CK20, S100, estrogen and progesterone receptors, and TTF-1. The second case is a 64-year-old female treated with vismodegib. Clinicopathological features and differential diagnoses are described.

Summary: Differentiating between basal cell carcinoma (BCC) and basaloid squamous cell carcinoma (bSCC) can be challenging both clinically and histologically. BCC is a common tumor with very rare metastasis, while bSCC is a rare entity with a higher propensity for distant metastasis. PD-1 inhibitors may be a valid therapeutic option for both types if they develop into inoperable metastatic disease.

The 10-year cumulative incidence of metastasis/death in BWH T2 BCC was 37%, indicating the need for surveillance for recurrence and clinical trials of adjuvant therapy.

Summary: Basal cell carcinomas (BCCs) are the most common malignant tumors in humans and are usually easily managed by surgery or topical therapies. However, metastatic BCCs are rare and may have distinct biological characteristics. Through genomic profiling, aberrant activation of Hedgehog signaling and alterations in multiple signaling pathways were identified in metastatic BCCs. The presence of clonal origin in matched local recurrences and metastases suggests that molecular profiling can assist in determining the nature and origin of poorly differentiated metastatic tumors. Dysregulation of the Hippo and PI3K/AKT pathways were implicated in the metastatic progression of BCCs, making them potential therapeutic targets.

Summary: The majority of non-melanoma skin cancer cases are basal cell carcinoma and squamous cell carcinoma which originate from keratinocytes. The incidence of keratinocyte carcinomas in the US is over 5 million per year, three times higher than all other types of cancer combined. Research has shown significant variations in gene expression in these tumors, with novel findings including up-regulation of zinc finger encoding genes in human BCC and a novel link between hedgehog signaling, Wnt signaling, and the cilium.

Summary: This study analyzed complications following cosmetic surgeries managed by plastic surgery services at Heatherwood and Wexham Park hospitals in 2022, including procedures performed internationally. The majority of complications were related to breast cosmetic surgery and abdominoplasties, with the most common complications being wound dehiscence, post-operative infection, and seromas. Turkey was the most popular country selected for surgery, and there is a need to increase public awareness of the risks involved in seeking international options and how to choose suitable clinics.

Summary: The study aims to compare the oncologic, medical, and surgical outcomes of lumpectomy and oncoplastic breast reduction surgery (OBRS) on a national scale. A database was used to analyze patient data and match them based on various factors. The results showed that lumpectomy patients had higher rates of repeat lumpectomy or subsequent mastectomy, while OBRS patients had higher rates of surgical complications but fewer medical complications. Logistic regression analysis revealed that OBRS was associated with a decreased likelihood of repeat lumpectomy. Although OBRS was associated with increased wound complications, medical complications were found to occur less frequently.
