Distinct Impact of Imatinib on Growth at Prepubertal and Pubertal Ages of Children with Chronic Myeloid Leukemia

Objective To determine the extent of growth impairment resulting from imatinib treatment in children with chronic myeloid leukemia (CML). Study design Clinical records of 48 chronic-phase CML children administered imatinib as the first-line therapy between 2001 and 2006 were analyzed retrospectively. Cumulative change in height was assessed using the height height-SDS and converted height data from age- and sex-adjusted Japanese norms. Results A decrease in height-SDS was observed in 72.9% of children, with a median maximum reduction in height-SDS of 0.61 during imatinib treatment. Median follow-up time was 34 months (range, 10-88 months). Growth impairment was seen predominantly in children who started imatinib at a prepubertal age compared with those who started at pubertal age. Growth velocity tended to recuperate in prepubertal children with growth impairment, as they reached pubertal age, suggesting that imatinib had little impact on growth during puberty. Conclusions Growth impairment was a major adverse effect of long-term imatinib treatment in children with CML. We report the distinct inhibitory effect of imatinib on growth in prepubertal and pubertal children with CML. We should be aware of growth deceleration in children, especially in young children given imatinib before puberty and subjected to prolonged exposure. (J Pediatr 2011;159:676-81).

This case may impact future RCC surveillance programs for patients with HLRCC.

A nationwide survey of late effects in survivors of juvenile myelomonocytic leukemia in Japan

Summary: In this study, a questionnaire was used to investigate the late effects in survivors of allogenic hematopoietic stem cell transplantation (HSCT) for juvenile myelomonocytic leukemia (JMML). The results showed that approximately 83% of the survivors experienced more than one late effect, including endocrine, dental, skin, ophthalmologic, musculoskeletal, pulmonary, neurocognitive, and cardiovascular dysfunction. The prevalence of short stature, pulmonary, cardiovascular, and nephrological complications was significantly higher among survivors who were more than 12 years post-HSCT. Therefore, establishing a multidisciplinary follow-up system for survivors of JMML is crucial.

Lung Cancer Complicated by Relapsing Polychondritis

Summary: A 77-year-old man presented with cough, pharyngeal discomfort, and weight loss for 1 month. Chest radiography showed a mass shadow in the right upper lung field. Bronchoscopy revealed multiple white nodules along the tracheal cartilage ring. Adenocarcinoma cells were detected in the mass, but no malignancy was found in several biopsy specimens of the tracheal lesions. F-18-fluorodeoxyglucose positron emission tomography showed intense accumulation in the mass, nasal septum, and tracheal cartilage. Furthermore, the patient had elevated levels of anti-type II collagen antibody. He was finally diagnosed with lung cancer complicated by relapsing polychondritis. The treatment included oral prednisolone and sequential chemoradiotherapy for lung cancer.

Summary: In this study, a novel pathogen detection technology called droplet digital PCR (ddPCR) was developed and compared with real-time quantitative PCR (qPCR) for detecting human adenovirus DNA. The results showed that ddPCR exhibited better reproducibility and sensitivity, as well as equivalent quantifiability, compared with qPCR. Therefore, ddPCR has the potential to provide a more accurate DNAemia detection, determine cutoff values for treatment initiation, and enable antiviral efficacy assessment.

Juvenile Hemochromatosis With Non-transfused Hemolytic Anemia Caused by a De Novo PIEZO1 Gene Mutation

Summary: Differential diagnosis of juvenile hemochromatosis with hemolytic anemia is challenging. We present a case of a 23-year-old woman with macrocytic hemolytic anemia and iron overload. A mutation in the PIEZO1 gene (heterozygous c.6008C>A: p.A2003D) was identified through target gene sequencing, which has been previously reported in a family with dehydrated hereditary stomatocytosis (DHS1, [OMIM 194380]). This case highlights the importance of considering DHS1 in the differential diagnosis of iron overload associated with non-transfused hemolytic anemia in children and young adults.

Health-Related Quality of Life for Parents of Infants with Bronchopulmonary Dysplasia

Summary: This prospective cohort study aimed to investigate the impact of bronchopulmonary dysplasia (BPD) on the health-related quality of life (HRQL) of infants from NICU hospitalization to one year post-discharge. The study found that lower HRQL during NICU stay was associated with earlier gestational age, postnatal corticosteroid usage, outborn status, and gastrostomy tube placement. Lower HRQL at 3 and 12 months post-discharge was associated with readmissions, home oxygen use, parent-reported difficulty breathing, lower developmental scores, and not playing with other children. Most parents reported similar or improved HRQL after discharge, but parents of infants with respiratory symptoms experienced less improvement. Efforts to improve parent HRQL should focus on respiratory symptoms and social isolation.

Reference Values for Cardiorespiratory Fitness in Patients Aged 6 to 18 Years

Early Elbow Flexion Contracture Predicts Shoulder Contracture in Infants with Brachial Plexus Birth Injury

Summary: This study aims to determine if children with elbow flexion contracture (EFC) caused by brachial plexus birth injury (BPBI) are more likely to develop shoulder contracture and undergo surgical treatment. A retrospective review was conducted on children under 2 years old with BPBI who presented to a single children's hospital. The results showed that patients with EFC had reduced shoulder range of motion and higher odds of shoulder contracture and surgical treatment. Prompt referral to a BPBI specialty clinic is recommended for evaluation and potential surgery.
