Prevalence of Congenital Heart Disease at Live Birth in Taiwan

Objectives To investigate the prevalence at live birth of congenital heart disease (CHD) in Taiwan. Study design Patients with CHD born from 2000 to 2006 were identified from National Health Insurance databases. Results CHD prevalence was 13.08 per 1000 live births: 12.05 (simple, 10.53; severe, 1.51) in male infants and 14.21 (simple, 12.90; severe, 1.32) in female infants. Ventricular septal defect (VSD; 4.0) was the most common defect, followed by secundum atrial septal defect (ASDII; 3.2), patent ductus arteriosus (PDA; 2.0), pulmonary stenosis (PS; 1.2), tetralogy of Fallot (TOF; 0.63), coarctation of aorta (CoA; 0.25), transposition of great arteries (TGA; 0.21), endocardial cushion defect (ECD; 0.20), double outlet of right ventricle (DORV; 0.15), total anomalous pulmonary venous return (TAPVR; 0.11), aortic stenosis (0.09), hypoplastic left heart syndrome (HLHS; 0.062), Ebstein anomaly (0.047), and tricuspid atresia (0.046). Female predominance was observed in VSD, ASDII, PDA, and ECD; and male predominance was observed in TGA and TOF. Ratios of western prevalence to our Asian prevalence were high for HLHS (3.68-4.5), CoA (1.13-1.96), TGA (1.09-1.83), and tricuspid atresia (1.09-2.57), but low for PS (0.15-0.99), TOF (0.41-0.92), and possibly ASDII. Conclusions In this Asian population, the prevalence of CHD was at the high end of the reported range, with more PS and TOF, but fewer left-sided obstructions, TGA, and tricuspid atresia. Perioperative outcomes of Fontan operation: Impact of heterotaxy syndrome

Summary: In this study, perioperative outcomes after Fontan operation in patients with heterotaxy syndrome (HS) were compared with other single-ventricular complex congenital heart diseases (SV-CHD). The results showed lower event-free survival and higher incidence of postoperative arrhythmia in the HS group. Preoperative high pulmonary artery pressure (PAP) and postoperative elevated central venous pressure (CVP) were identified as risk factors for poor outcomes.

Clinical implications of congenital interruption of inferior vena cava

Summary: Inferior vena cava (IVC) interruption can present as an isolated lesion and is associated with congenital heart disease (CHD). Bradycardia is common among patients, but the severity of CHD is the only risk factor for survival. Patients with IVC interruption often have left isomerism at the atrial and bronchopulmonary situs, while the spectrum of splenic abnormalities is wide, including polysplenia, lobulated spleen, solitary inversus spleen, and rarely, asplenia.

Summary: The role of preoperative N-terminal pro-brain natriuretic peptide level in patient outcomes after the Fontan operation was investigated in this study. The results showed that high preoperative N-terminal pro-brain natriuretic peptide was associated with poor outcomes after the operation.

Summary: This study aimed to explore the lived experience of first-time mothers with congenital heart disease (CHD). The findings showed that recognizing pregnancy risks, performing self-care for health, building self-worth from their baby, adapting to postpartum life and adjusting priorities, enjoying being a first-time mother, and the factors contributing to success in high-risk childbirth were the main themes influencing the experiences of these women.

Pulmonary Hypertension in Adult Congenital Heart Disease in Asia: A Distinctive Feature of Complex Congenital Heart Disease

Summary: This study investigates the clinical characteristics of pulmonary hypertension (PH) in adult congenital heart disease (CHD) patients in Taiwan, with a focus on complex CHD. The cumulative incidence of PH in this Asian cohort was similar to that in Western countries. Patients with PH had higher mortality rates, but survival rates were similar across different CHD subtypes.

The study included all patients aged <18 years who received VAD implantation between 2008 and 2021. The results showed a survival rate of 72.7% at 1 month, 67.7% at 3 months, and 67.7% at 6 months post-implantation.

Management of cardiovascular symptoms after Pfizer-BioNTech COVID-19 vaccine in teenagers in the emergency department

Summary: This study analyzed the risk factors for cardiovascular complications after Pfizer-BioNTech COVID-19 vaccination in adolescents. It found that 7.2% of patients aged 12-18 years were diagnosed with pericarditis/myocarditis, with chest pain being the most common symptom. Electrocardiography was found to be a reliable screening tool for myocarditis.

Self-Expanding Pulmonary Valves in 53 Patients With Native Repaired Right Ventricular Outflow Tracts

Summary: This study aimed to evaluate the efficacy of percutaneous pulmonary valve implantation (PPVI) on right ventricular (RV) function and graft remodeling in patients with native repaired right ventricular outflow tracts. The results showed that within 6 months after PPVI, all patients exhibited resolution of paradoxical septal motion and a significant reduction in RV volume, N-terminal pro-B-type natriuretic peptide levels, and valve eccentricity indices. However, only 9 patients (17.3%) achieved normalization of the RV ejection fraction (> 50%).

Summary: The purpose of this study is to determine the ECG parameter change and the efficacy of ECG screening for cardiac adverse effect after the second dose of BNT162b2 vaccine in young population. The study found that cardiac symptoms were common after the second dose, with chest pain and palpitations being the most reported. However, the incidences of significant arrhythmias and myocarditis were low. The serial ECG screening method showed high sensitivity and specificity for significant cardiac adverse effects.

Health-Related Quality of Life for Parents of Infants with Bronchopulmonary Dysplasia

Summary: This prospective cohort study aimed to investigate the impact of bronchopulmonary dysplasia (BPD) on the health-related quality of life (HRQL) of infants from NICU hospitalization to one year post-discharge. The study found that lower HRQL during NICU stay was associated with earlier gestational age, postnatal corticosteroid usage, outborn status, and gastrostomy tube placement. Lower HRQL at 3 and 12 months post-discharge was associated with readmissions, home oxygen use, parent-reported difficulty breathing, lower developmental scores, and not playing with other children. Most parents reported similar or improved HRQL after discharge, but parents of infants with respiratory symptoms experienced less improvement. Efforts to improve parent HRQL should focus on respiratory symptoms and social isolation.

Reference Values for Cardiorespiratory Fitness in Patients Aged 6 to 18 Years

Early Elbow Flexion Contracture Predicts Shoulder Contracture in Infants with Brachial Plexus Birth Injury

Summary: This study aims to determine if children with elbow flexion contracture (EFC) caused by brachial plexus birth injury (BPBI) are more likely to develop shoulder contracture and undergo surgical treatment. A retrospective review was conducted on children under 2 years old with BPBI who presented to a single children's hospital. The results showed that patients with EFC had reduced shoulder range of motion and higher odds of shoulder contracture and surgical treatment. Prompt referral to a BPBI specialty clinic is recommended for evaluation and potential surgery.
