Objective. To investigate the increased risk of congenital, neurologic, and endocrine disorders in autistic preschool children and to probe possible cognitive impairment-associated variation in such risks. Study design Using a population-based longitudinal study, a total of 3440 autistic children born in 1997-1999 and 33 391 age-. and residential urbanicity-matched control subjects; were identified from the National Health Insurance Research Database in Taiwan. Conditional logistic analyses were performed to estimate the strength of association stratified by the presence of cognitive impairment. Results Autistic children were found to have greatly elevated risks of congenital anomalies (eg, tuberous sclerosis: adjusted odds ratio [aOR] = 34 similar to 61) and neurologic disorders (eg, epilepsy: aOR = 5 similar to 13) compared with their matched nonautistic peers. The increased risk of medical diseases for mentally retarded in autism were approximately 1.6 to 9 times greater than those for isolated autism. Conclusions The observed cognitive impairment-related variation in the increased risk of congenital. neurological, and endocrine disorders with autism may provide some clinical and etiologic implications that warrant investigation in the future. Summary: This study investigated the influencing factors of family caregivers' readiness for discharge in advanced cancer patients. The findings showed that family caregivers' readiness for discharge affects patients' ability to be discharged home. Factors influencing readiness included the patient's physical activity performance status and expressed discharge willingness, the presence of someone to assist with home care, and the difficulties of providing in-home care.

Frailty and Quality of Life among Older Adults in Communities: The Mediation Effects of Daily Physical Activity and Healthy Life Self-Efficacy

Chia-Hui Lin, Chieh-Yu Liu, Chun-Ching Huang, Jiin-Ru Rong

Summary: This study examined the mediating effects of daily physical activity and healthy life self-efficacy on the relationship between frailty and quality of life in older adults. The findings suggest that daily physical activity and healthy life self-efficacy partially mediate the relationship between frailty and quality of life. Additionally, serial mediation analysis showed that the association between frailty and quality of life is partially mediated by daily physical activity and healthy life self-efficacy in a sequential manner.

The effect of research on life satisfaction in middle-aged and older adults: physical disability and physical activity as a parallel and serial mediation analysis

Pei-Shan Li, Chia-Jung Hsieh, Ya-Ling Shih, Ya-Ting Lin, Chieh-Yu Liu

Summary: This study aimed to explore the impact of frailty in middle-aged and older adults on life satisfaction under the simultaneous occurrence and correlation of physical disability and physical activity status. The results showed that physical disability and physical activity partially mediated the relationship between frailty status and life satisfaction. Frailty led to increased physical disability, which in turn affected physical activity and led to lower life satisfaction. Stratifications by age significantly increased the mediating effect of physical activity but did not reduce the mediating effect of physical disability.

Summary: Self-care and self-efficacy are important factors for predicting the quality of life in hemodialysis patients. However, there is currently a lack of a Vietnamese instrument for assessing these factors. This study aimed to validate and assess the reliability of the Vietnamese version of the Strategies Used by People to Promote Health questionnaire. The questionnaire showed good content validity and reliability, indicating its usefulness for measuring self-care and self-efficacy in hemodialysis patients.

Health-Related Quality of Life for Parents of Infants with Bronchopulmonary Dysplasia

Kathryn E. K. Berlin, William Scott, Sara Dawson, David Brousseau, Joanne M. Lagatta

Summary: This prospective cohort study aimed to investigate the impact of bronchopulmonary dysplasia (BPD) on the health-related quality of life (HRQL) of infants from NICU hospitalization to one year post-discharge. The study found that lower HRQL during NICU stay was associated with earlier gestational age, postnatal corticosteroid usage, outborn status, and gastrostomy tube placement. Lower HRQL at 3 and 12 months post-discharge was associated with readmissions, home oxygen use, parent-reported difficulty breathing, lower developmental scores, and not playing with other children. Most parents reported similar or improved HRQL after discharge, but parents of infants with respiratory symptoms experienced less improvement. Efforts to improve parent HRQL should focus on respiratory symptoms and social isolation.

Early Elbow Flexion Contracture Predicts Shoulder Contracture in Infants with Brachial Plexus Birth Injury

David S. Liu, Patricia Miller, Anna Rothenberg, Carley Vuillermin, Peter M. Waters, Andrea S. Bauer

Summary: This study aims to determine if children with elbow flexion contracture (EFC) caused by brachial plexus birth injury (BPBI) are more likely to develop shoulder contracture and undergo surgical treatment. A retrospective review was conducted on children under 2 years old with BPBI who presented to a single children's hospital. The results showed that patients with EFC had reduced shoulder range of motion and higher odds of shoulder contracture and surgical treatment. Prompt referral to a BPBI specialty clinic is recommended for evaluation and potential surgery.
