Summary: This study investigated predictors of PA-adherent lymph nodes and conversion/PA injury during minimally invasive surgery. Bronchoscopy and CT were found to be independent predictors, with the combination of dark pigmentation and LN size useful in stratifying patients' risk of PA-adherent LNs. The presence of PA-adherent LNs was associated with a higher risk of conversion and PA injury during MIS.

The developed Essen score can accurately predict the occurrence of postoperative complications.

Summary: This comparative study evaluated the efficacy of poloxamer 407-based ropivacaine hydrogel (Gel) at the wound site versus continuous thoracic paravertebral block using On-Q PainBuster for postoperative pain after thoracoscopic pulmonary resection. The study found that Gel was noninferior to On-Q in terms of fentanyl consumption, pain intensity, and the need for rescue analgesia. The local injection of Gel was considered to be a viable alternative to On-Q for regional analgesia due to its technical simplicity and low systemic toxicity.

The results showed that with experience and communication, select patients can be discharged on postoperative day one with excellent outcomes and high satisfaction. Discharge on the first day was associated with never smokers and segmentectomy, while a decreased baseline performance status and perioperative complications were inversely associated with discharge on the first day.

Summary: The development of advanced robotic platforms has led to increased adoption of minimally invasive approaches in thoracic surgery. This study analyzed trends in thoracic approaches using data from the National Cancer Database from 2010 to 2018. The results showed a decrease in open resections and an increase in minimally invasive approaches, particularly robotic resection. Tumors requiring radiation therapy were more likely to be treated with minimally invasive surgery. Factors such as race, hospital volume, insurance, and academic setting were associated with the likelihood of minimally invasive surgery.
