Pediatric pancreatitis has received much attention during the past few years. Numerous reports have identified an increasing trend in the diagnosis of acute pancreatitis in children and key differences in disease presentation and management between infants and older children. The present review provides a brief, evidence-based focus on the latest progress in the clinical field. It also poses important questions for emerging multicenter registries to answer about the natural history and management of affected children with pancreatitis.

Summary: Chronic pancreatitis is rare but impactful in childhood, and surgical management must be tailored to the patient's unique needs, considering the morphology of the disease and presence of genetic risk factors. Total pancreatectomy with islet autotransplantation is the best surgical option for patients with small duct disease.

Summary: Endoscopic retrograde cholangiopancreatography (ERCP) is commonly used for pancreaticobiliary disorders, but it may lead to post-ERCP pancreatitis (PEP). Current research suggests that the calcineurin inhibitor tacrolimus has the potential to prevent PEP. This review summarizes the role of tacrolimus in PEP prophylaxis and explores the advantages of rectal administration.

Clinical insights into drug-associated pancreatic injury

Summary: This review challenges the traditional concept of drug-induced pancreatitis as a diagnosis of exclusion, proposing instead a continuum of pancreatic injury termed drug-associated pancreatic injury (DAPI) that can occur concomitantly with other risk factors. The review aims to build a foundation for conceptualizing DAPI, highlight specific drugs associated with DAPI, and provide a framework for future studies.

Summary: Pancreas Divisum (PD) is a common pancreatic ductal variant that is more frequently observed in pediatric patients with acute recurrent pancreatitis (ARP) compared to the general population. Endoscopic retrograde cholangiopancreatography (ERCP) with minor papillotomy has been shown to be beneficial in relieving symptoms and improving clinical outcomes in these patients.

Summary: A systems approach revealed that vitamin A can reduce the risk of asparaginase-associated pancreatitis (AAP). Lower levels of vitamin A were found in ALL patients who developed pancreatitis. Therefore, vitamin A supplementation should be considered in preventing or treating AAP.

Summary: This study aimed to analyze the characteristics, co-risk factors, and reliability of the Naranjo scoring system for drug-associated acute pancreatitis (DAP) in children with acute recurrent pancreatitis (ARP) and chronic pancreatitis (CP). The results showed that medications were involved in 7% of ARP or CP patients in the INSPPIRE-2 cohort, with most patients having other risk factors for pancreatitis. The Naranjo scoring system raised questions about its reliability for DAP.
