Wound Closure Following Intervention for Closed Orthopedic Trauma

Matthew J. Gotlin, William Catalano, Jamie P. Levine, Kenneth A. Egol

Summary: The method of skin closure and post-operative wound management is crucial in orthopedic surgery, especially with the increasing importance of surgical site infection as a healthcare metric. Understanding the physiology of wound healing and factors affecting healing potential is essential to minimize complications. The goal of skin closure techniques is to promote rapid healing with minimal risk of infection and dehiscence. Knowledge of closure materials, techniques, dressings, and local therapies is important for optimizing wound healing.

Double-Barrel Versus Single-Barrel Fibula Flaps for Mandibular Reconstruction: Safety and Outcomes

Jorge Trilles, Bachar F. Chaya, David A. Daar, Lavinia Anzai, Daniel Boczar, Ricardo Rodriguez Colon, David L. Hirsch, Adam S. Jacobson, Jamie P. Levine

Summary: This retrospective study compared the outcomes of single-barrel and double-barrel fibula flaps in mandibular reconstruction for head and neck defects. The results showed that double-barrel reconstruction did not increase postoperative morbidity or duration of hospital stay compared to single-barrel reconstruction. Additionally, double-barrel reconstruction was associated with higher odds of successful primary dental implantation.

Robotic deep inferior epigastric perforator flap harvest in breast reconstruction

David A. Daar, Lavinia M. Anzai, Neil M. Vranis, Michael L. Schulster, Jordan D. Frey, Min Jun, Lee C. Zhao, Jamie P. Levine

Summary: This study describes a minimally-invasive technique for robotic deep inferior epigastric artery perforator (DIEP) flap harvest and presents data from four patients who underwent a total of seven robotic-assisted DIEP flap operations. The results demonstrate the feasibility and safety of this technique.

Robotic-Assisted Testicular Autotransplantation

Brian W. Chao, Nabeel A. Shakir, Grace S. Hyun, Jamie P. Levine, Lee C. Zhao

Summary: Robotic-assisted testicular autotransplantation is a feasible and effective management option for solitary intra-abdominal testis. The procedure preserves testicular endocrine function and eliminates the need for intraoperative testicular biopsy.

Early-onset osteoradionecrosis following adjuvant volumetric-modulated arc therapy to an osteocutaneous free fibula flap with customized titanium plate

David A. Daar, David J. Byun, Karl Spuhler, Lavinia Anzai, Lukasz Witek, David Barbee, Kenneth S. Hu, Jamie P. Levine, Adam S. Jacobson

Summary: Computerized surgical planning (CSP) is crucial in osseous reconstruction of head and neck cancer defects, but the impact of CSP-designed titanium plating systems on radiation therapy remains unclear, and there may be an underreported risk of osteoradionecrosis (ORN) and local recurrence.

Breast reconstruction during the COVID-19 pandemic: Single institution experience from the pandemic's epicenter in the United States

Carter J. Boyd, Kshipra Hemal, Sruthi Ramesh, Jonathan M. Bekisz, Ara A. Salibian, Vishal Thanik, Jamie P. Levine, Mihye Choi, Nolan S. Karp

Summary: This study aimed to quantify the impact of the COVID-19 pandemic on breast cancer screening, treatment, and reconstruction at a medical institution in New York City. The results showed a decline in mammography, oncologic breast surgeries, and breast reconstruction in 2020, but a rebound in 2021. Changes in the types of reconstruction were also noted.

Free Fibula Flap for the Treatment of Agnathia in a 10-Year-Old With Severe Agnathia-Otocephaly Complex

Oriana Cohen, Kerry A. Morrison, Adam Jacobson, Jamie Levine, David A. Staffenberg

Summary: This study presents the technical considerations and details of mandibular reconstruction using virtual surgical planning and a vascularized free fibula flap in a 10-year-old boy with agnathia-otocephaly complex, a rare and often fatal condition. Virtual surgical planning was utilized to plan the reconstruction of the mandibular symphysis and bilateral body segments, along with the use of bilateral costochondral grafts for the rami. This is the first application of virtual surgical planning for mandibular reconstruction in a pediatric patient with severe agnathia-otocephaly complex.
