Divergent associations of plasma choline and betaine with components of metabolic syndrome in middle age and elderly men and women

Choline is involved in the synthesis of phospholipids, including blood lipids, and is the immediate precursor of betaine, which serves as a methyl group donor in a reaction converting homocysteine to methionine. Several cardiovascular risk factors are associated with plasma homocysteine, whereas little is known about their relationship to choline and betaine. We examined the relation of plasma choline and betaine to smoking, physical activity, BMI, percent body fat, waist circumference, blood pressure, serum lipids, and glucose in a population-based study of 7074 men and women aged 47-49 and 71-74 y. Overall plasma concentrations (means +/- SD) were 9.9 +/- 2.3 mu mol/L for choline and 39.5 +/- 12.5 mu mol/L for betaine. Choline and betaine were lower in women than in men and in younger subjects compared with older (P<0.0001). Multivariate analyses showed that choline was positively associated with serum triglycerides, glucose, BMI, percent body fat, waist circumference (P < 0.0001 for all), and physical activity (P < 0.05) and inversely related to HDL cholesterol (P < 0.05) and smoking (P < 0.0001). Betaine was inversely associated with serum non-HDL cholesterol, triglycerides, BMI, percent body fat, waist circumference, systolic and diastolic blood pressure (P < 0.0001 for all), and smoking (P < 0.05) and positively associated with HDL cholesterol (P < 0.01) and physical activity (P < 0.0001). Thus, an unfavorable cardiovascular risk factor profile was associated with high choline and low betaine concentrations. Choline and betaine were associated in opposite directions with key components of metabolic syndrome, suggesting a disruption of mitochondrial choline dehydrogenase pathway.

Effectiveness and safety of non-vitamin K antagonist oral anticoagulants and warfarin in atrial fibrillation: a Scandinavian population-based cohort study

Summary: This study evaluated the risk of stroke/systemic embolism and bleeding in anticoagulation-naive patients with atrial fibrillation using non-vitamin K antagonist oral anticoagulants compared to warfarin. The results showed that patients initiating dabigatran, apixaban, and rivaroxaban had similar rates of stroke/systemic embolism to patients initiating warfarin. Rates of major bleeding were lower with apixaban and dabigatran, and higher with rivaroxaban, compared to warfarin.

Nine-fold higher risk of acute myocardial infarction in subjects with type 1 diabetes compared to controls in Norway 1973-2017

Summary: This study aimed to investigate the incidence and risk factors of acute myocardial infarction (AMI) in patients with type 1 diabetes. The findings showed a nine-fold increased risk of AMI in subjects with type 1 diabetes compared to controls, and a three-fold higher risk in individuals with lower education. These results highlight the importance of prevention of cardiovascular disease and personalized diabetes education based on patients' educational background.

Summary: The association between the prescription-based Rx-Risk Comorbidity Index and mortality by history of hip fracture was studied in a prospective study using nationwide registry data in Norway. It was found that history of hip fracture and comorbidity assessed by Rx-Risk are independent risk factors for mortality in the community-dwelling older population in Norway, with comorbidity explaining a quarter of the excess mortality in persons with a history of hip fracture.

Summary: Women with pre-eclampsia have an increased risk of cardiovascular disease (HR 2.16), especially when accompanied by a large for gestational age offspring (HR 2.57). Diabetes during pregnancy also increases the risk of CVD, but it is not affected by offspring weight.

Inflammation, sex, blood pressure changes and hypertension in midlife: the Hordaland Health Study

Summary: The aim of this study was to examine the associations between sex-specific circulating markers of inflammation and blood pressure in midlife. The results showed that higher levels of hs-CRP were significantly associated with higher systolic and diastolic blood pressure in women, but not in men. Additionally, higher levels of neopterin were associated with higher diastolic blood pressure in women, and higher PAr index was associated with higher blood pressure in both men and women.

Plasma methylmalonic acid predicts risk of acute myocardial infarction and mortality in patients with coronary heart disease: A prospective 2-cohort study
