Generating Evidence for Clinical Benefit of PET/CT in Diagnosing Cancer Patients

For diagnostic methods such as PET/CT, not only diagnostic accuracy but also clinical benefit must be demonstrated. However, there is a lack of consensus about how to approach this task. Here we consider 6 clinical scenarios to review some basic approaches to demonstrating the clinical benefit of PET/CT in cancer patients: replacement of an invasive procedure, improved accuracy of initial diagnosis, improved accuracy of staging for curative versus palliative treatment, improved accuracy of staging for radiation versus chemotherapy, response evaluation, and acceleration of clinical decisions. We also develop some guidelines for the evaluation of clinical benefit. First, it should be clarified whether there is a direct benefit of the use of PET/CT or an indirect benefit because of improved diagnostic accuracy. If there is an indirect benefit, then decision modeling should be used initially to assess the benefit expected from the use of PET/CT. Only if decision modeling does not allow definitive conclusions should randomized controlled trials be planned.

Summary: This study investigated whether surgeon-performed ultrasonography is as effective as scintigraphy in localizing pathological parathyroid glands in patients with primary hyperparathyroidism. The results showed that conventional ultrasonography by an experienced parathyroid surgeon-sonographer was noninferior to scintigraphy and may be considered as a valid first-line imaging modality.

Is there more than meets the eye in PSMA imaging in prostate cancer with PET/MRI? Looking closer at uptake time, correlation with PSA and Gleason score

Summary: This study aimed to determine the effects of different PSMA PET timing on signal kinetics and its correlation with patients' PSA and Gleason scores. The results showed that the detection of tumors remained unchanged in static and dynamic scans. The activity concentration trend of lesions in pelvic lymph nodes and bones was mostly negative, while lesions in the prostate and prostatic fossa showed a positive trend. The correlation between dynamic parameters and PSA varied depending on the radiotracer used.

Modeling Bland-Altman Limits of Agreement with Fractional Polynomials-An Example with the Agatston Score for Coronary Calcification

Summary: Bland-Altman limits of agreement are commonly used in method comparison studies for quantitative outcomes. However, certain assumptions need to be met for a straightforward application of Bland-Altman analysis. If these assumptions are violated, variance-stabilizing transformations or fractional polynomial regression can be used. This review revisits an inter-rater agreement analysis on coronary calcification and demonstrates the inappropriate use of a straightforward Bland-Altman analysis, as well as the application of nonparametric limits of agreement and fractional polynomials.

Gwet's AC1 is not a substitute for Cohen's kappa- A comparison of basic properties

Summary: Gwet's AC1 is proposed as an alternative to Cohen's kappa for evaluating agreement between binary ratings. This paper investigates the properties of Gwet's AC1 and compares it to Cohen's kappa, particularly in terms of the impact of prevalence of positive ratings and behavior in cases of no association or maximal disagreement. The two approaches have fundamental differences in how they are calculated and interpreted, making Gwet's AC1 not a substitute for Cohen's kappa.

Summary: This study investigated the diagnostic accuracy of up-front 18F-fluorodeoxyglucose positron emission tomography/computed tomography (PET/CT) and MRI for detecting cervical lymph node metastases in patients with T1-T2 oral squamous cell carcinoma. The results showed that PET/CT had higher sensitivity but lower specificity compared to neck MRI, and the overall accuracy of both imaging modalities was comparable. Therefore, a combination of PET/CT and MRI should be used for diagnosing cervical lymph node metastases in T1-T2 oral cavity cancer.
