Molecular imaging uses noninvasive techniques to visualize various biologic pathways and physiologic characteristics of tumors and normal tissues. In relation to radiation therapy, PET with the tracer F-18-FDG offers a unique opportunity to refine the target volume delineation in patients with squamous cell carcinoma of the head and neck, in turn affecting dose distribution and, it is hoped, patient outcome. Even more so, in the framework of adaptive treatment and theragnostics, whereby dose distribution is adapted in space and time over the typical course of radiotherapy, molecular imaging with PET offers an elegant research avenue to further improve the therapeutic ratio. Such implementation could be of particular interest with tracers other than F-18-FDG, such as tracers of hypoxia and proliferation.

Advanced Imaging Supports the Mechanistic Role of Autoimmunity and Plaque Rupture in COVID-19 Heart Involvement

Maria Elena Laino, Angela Ammirabile, Francesca Motta, Maria De Santis, Victor Savevski, Marco Francone, Arturo Chiti, Lorenzo Mannelli, Carlo Selmi, Lorenzo Monti

Summary: The cardiovascular system is frequently affected by COVID-19, particularly in hospitalized cases, leading to complications such as myocarditis and pulmonary embolism. Early detection through imaging techniques such as CT scan and MRI is important for prompt disease recognition and evaluation. The immune system's abnormal response is responsible for severe COVID-19 and organ-specific injury, with molecular mimicry possibly being the immunological mechanism for myocarditis in COVID-19.

The Relationship among Bowel [18]F-FDG PET Uptake, Pathological Complete Response, and Eating Habits in Breast Cancer Patients Undergoing Neoadjuvant Chemotherapy

Summary: The impact of patients' eating habits on breast cancer management and inflammation has been investigated. The study found positive correlations between bowel [18]F-FDG uptake and pro-inflammatory drinks and foods, as well as a negative correlation between rectum SUVmean and anti-inflammatory foods. In addition, colon SUVmean was significantly lower in patients with pCR. This suggests that PET scan could be used to identify patients with unhealthy behaviors.

The Role of PET Imaging in the Differential Diagnosis between Radiation Necrosis and Recurrent Disease in Irradiated Adult-Type Diffuse Gliomas: A Systematic Review

Gaia Ninatti, Cristiano Pini, Fabrizia Gelardi, Martina Sollini, Arturo Chiti

Summary: This study aimed to review the use of PET imaging in adult-type diffuse gliomas and assess its capability in distinguishing radiation necrosis from tumor recurrence. The study found that PET imaging has some value in differentiating between these two conditions, but there is high heterogeneity among existing studies.

The complementary role of PSMA expression and [18F]FDG PET/CT in predicting thyroid cancer outcome: from black and white to shades of gray, in the era of precision oncology

Summary: This study investigated the potential role of Prostate Specific Membrane Antigen (PSMA) in thyroid carcinoma (TC) and found that PSMA expression and 2-[F-18]fluoro-2-deoxy-D-glucose ([F-18]FDG) uptake on PET/CT can serve as complementary biomarkers for TC outcome prediction.

Internal Guidelines for Reducing Lymph Node Contour Variability in Total Marrow and Lymph Node Irradiation

Damiano Dei, Nicola Lambri, Sara Stefanini, Veronica Vernier, Ricardo Coimbra Brioso, Leonardo Crespi, Elena Clerici, Luisa Bellu, Chiara De Philippis, Daniele Loiacono, Pierina Navarria, Giacomo Reggiori, Stefania Bramanti, Marcello Rodari, Stefano Tomatis, Arturo Chiti, Carmelo Carlo-Stella, Marta Scorsetti, Pietro Mancosu

Summary: The contouring of lymph node clinical target volume (CTV_LN) in total marrow and lymph node irradiation (TMLI) is complex and time-consuming. The introduction of internal guidelines reduced inter- and intraobserver variability in CTV_LN delineation. The guidelines improved the consistency of contouring in TMLI treatments.

The potential role of osteoporosis in unspecific [18F]PSMA-1007 bone uptake

Gaia Ninatti, Cristiano Pini, Fabrizia Gelardi, Samuele Ghezzo, Paola Mapelli, Maria Picchio, Lidija Antunovic, Alberto Briganti, Francesco Montorsi, Claudio Landoni, Martina Sollini, Arturo Chiti

Summary: In this study, we investigated the potential association between osteoporosis and unspecific bone uptake in PET imaging using [F-18]PSMA-1007. We found that blood count parameters showed a significant correlation with unspecific bone uptake, suggesting a possible link between [F-18]PSMA-1007 uptake and osteoporosis. Further research with larger populations and specific osteoporosis markers is needed to explore this hypothesis.

The prognostic power of [11C]methionine PET in IDH-wildtype diffuse gliomas with lower-grade histological features: venturing beyond WHO classification

Gaia Ninatti, Cristiano Pini, Beatrice Claudia Bono, Fabrizia Gelardi, Lidija Antunovic, Bethania Fernandes, Martina Sollini, Claudio Landoni, Arturo Chiti, Federico Pessina

Summary: This study aimed to assess the prognostic value of MET PET in newly diagnosed, treatment naive IDH-wt gliomas with histological features of LGGs. The results showed that positive MET PET was significantly associated with shorter PFS and OS time. Incomplete surgical resection and higher TBRmean values were independent predictors of shorter PFS, while higher tumor grade and incomplete surgical resection were independent predictors of OS.

Imaging in Gastroparesis: Exploring Innovative Diagnostic Approaches, Symptoms, and Treatment

Francesco Vito Mandarino, Sabrina Gloria Giulia Testoni, Alberto Barchi, Francesco Azzolini, Emanuele Sinagra, Gino Pepe, Arturo Chiti, Silvio Danese

Summary: Gastroparesis is a chronic disease characterized by upper gastrointestinal symptoms and delayed gastric emptying. The gold standard for diagnosis is gastric emptying scintigraphy, although its correlation with symptoms is weak. Recent research focuses on developing treatments that target the underlying pathophysiology, such as Gastric Peroral Endoscopic Myotomy. New diagnostic tools, including wireless motility capsule and the Endoluminal Functional Lumen Imaging Probe, have the potential to characterize the pathophysiology of gastroparesis and identify patients who may benefit from targeted therapies.

Summary: This study proposes a new workflow for accurate co-registration of prostate PET images with histopathological images using a dual-tracer PET/MRI study. The method allows a deeper understanding of PET tracer distribution and enables advanced analyses. The co-registration workflow involves five steps and the precision was evaluated using target registration error (TRE) and DICE score.

COVID-19 vaccination, implications for PET/CT image interpretation and future perspectives

Margarita Kirienko, Matteo Biroli, Cristiano Pini, Fabrizia Gelardi, Martina Sollini, Arturo Chiti

Summary: PET/CT scans can identify COVID-19 vaccine-related deltoid [F-18]FDG uptake and axillary hypermetabolic lymph nodes, with variable frequency depending on the tracer used. Awareness of vaccination timing, patient characteristics, and concurrent therapies is necessary for interpreting vaccine-related findings.
