Summary: This study retrospectively reviewed a case series of endoscopic endonasal biopsies of isolated cavernous sinus lesions at two tertiary institutions. The results showed that endoscopic endonasal biopsy was safe and effective in obtaining histologic diagnosis for these lesions, and had a significant impact on therapeutic decision making.

Summary: The aim of this study was to determine if the distinction between planum sphenoidale (PS) and tuberculum sellae (TS) meningiomas is clinically meaningful and impacts the results of the endoscopic endonasal approach (EEA). The study found that PS meningiomas were larger and more likely to invade the optic canals. The surgical outcomes for both PS and TS meningiomas were similar.

Summary: Visual deterioration after endoscopic endonasal transsphenoidal surgery (EETS) is a rare but serious complication that can be effectively treated if diagnosed and intervened in a timely manner. This study analyzed 1200 cases of EETS and found that early postoperative visual deterioration is most commonly associated with craniopharyngioma. Timely intervention restored vision in 81% of patients.

Summary: The endoscopic endonasal approach for patients with a sellar tumor is a gentle method with minimal impact on sinonasal quality of life over a period longer than six months. Common complaints post-surgery include the need to blow nose, nasal congestion, loss of smell and taste, and thick discharge from the nose. Cranial base reconstruction using muscle and fascia lata appears to positively influence sinonasal quality of life.

A newly proposed resectability score was found to successfully predict the likelihood of gross total resection (GTR). Long-term recurrence rates varied significantly based on the extent of resection, with GTR significantly reducing recurrence compared to near-total resection (NTR) and subtotal resection (STR).

Summary: This study investigated the efficacy of a new ultra-short antibiotic prophylaxis protocol for endoscopic endonasal skull base surgery, guided by preoperative microbiological nasal swab results. The results showed that the nasal swab-related antibiotic protocol effectively prevented infections, with no cases reported in the case group while two infections occurred in the control group. Although the new protocol was more expensive than the standard hospital antibiotic protocol used in neurosurgery, it was found to be cheap compared to other antibiotic prophylaxis protocols reported in literature.

Summary: The objective of this study was to determine the economic feasibility of penicillin allergy testing in preventing unnecessary clindamycin use among patients with an unconfirmed penicillin allergy prior to otolaryngologic surgery. The results showed that preoperative penicillin testing was economically sustainable if it could decrease the CDI rate by 1.06% or the SSI rate by 1.34%. The study suggests modifying current practice guidelines to recommend penicillin allergy testing in patients with an unconfirmed allergy prior to surgery.
