Summary: A pituitary incidentaloma is a pituitary tumor or mass that is incidentally discovered in imaging studies for reasons unrelated to pituitary lesions. The majority of these tumors are PitNETs and Rathke cleft cysts. The management of pituitary incidentalomas is controversial, with surgical resection recommended for clinically non-functioning PitNETs with optic chiasm compression, while cystic lesions like Rathke cleft cysts should be followed if the patients are asymptomatic. The pathogenesis of PitNETs has changed with the new WHO classification, highlighting the need for evidence-based research to determine treatment guidelines for pituitary incidentalomas in the future.
