Summary: This study examined the risk factors associated with leptomeningeal disease (LMD) in patients who underwent resection of a brain metastasis. The results showed that tumor location, tumor volume, extracranial disease status, and ventricle contact were associated with LMD.

Summary: This study evaluated central nervous system involvement in patients with neuroendocrine neoplasms, particularly focusing on leptomeningeal carcinomatosis (LC). LC is associated with poor prognosis, and optimal treatment strategies are needed. Common symptoms include motor and sensory deficits as well as vigilance abnormalities.

Summary: This study described impairments and rehabilitation utilization among patients with cytology-proven LMD receiving intrathecal chemotherapy, revealing that the majority of patients had physical impairments which could affect their physical function, but most were able to avoid post-acute inpatient rehabilitation through rehabilitation services. Additional research and education for rehabilitation professionals about this increasingly common syndrome are needed.

Summary: This study aims to determine if surgical or anatomic factors can predict the occurrence of leptomeningeal disease (LMD) in patients with metastatic melanoma. The results showed that factors such as female gender, dural-based and intraventricular metastasis, and tumor bordering cerebrospinal fluid (CSF) spaces were associated with an increased risk of LMD. However, surgical resection alone did not have a significant impact on the risk of LMD.

Summary: CSF diversion (VP or LP shunt) is an effective and safe treatment for lung cancer patients with LMC and hydrocephalus, potentially increasing overall survival and improving quality of life. Programmable shunts should be considered for complex cases to allow for pressure adjustments as the disease progresses.

Summary: This study aimed to evaluate the outcomes and prognostic factors of patients with non-small cell lung cancer (NSCLC) with leptomeningeal metastasis (LM) concurrent or after brain metastasis (BM). The study found that patients without BM had better survival outcomes, and favorable prognostic factors included a Karnofsky Performance Status (KPS) score ≥70 and shunt implantation.

Summary: Leptomeningeal carcinomatosis (LC) is a rare complication of metastatic breast cancer, occurring in only 5% of patients, but often presenting with severe neurological deficits. Patients typically have a median survival of only 10 to 15 weeks, making the progressive neurologic dysfunction fatal.

Summary: This study evaluated the real-world local recurrence of all brain metastasis patients referred for adjuvant stereotactic radiosurgery (SRS) and identified predictors of SRS timing. The timing of SRS was found to be associated with local recurrence rate and LRFS, with receiving SRS within 4 weeks postoperatively potentially reducing the recurrence rate.

Summary: This review provides an overview of the role of extracellular matrix (ECM) in brain metastasis. It discusses the involvement of ECM in disrupting the blood-brain barrier, modulating brain metastasis dormancy, regulating brain metastasis seeding through activated integrin signaling, and the functions of brain-specific ECM protein reelin in brain metastasis. The possibility of targeting ECM for brain metastasis management is also considered.

Summary: This study analyzed the incidence and risk factors of leptomeningeal metastasis (LM) development in patients with non-small cell lung cancer (NSCLC) and brain metastases (BMs) after stereotactic radiosurgery (SRS), as well as the survival outcomes and prognostic factors after LM development. The results showed that LM developed in 13.7% of NSCLC patients after SRS treatment. Large initial tumor volume and more than 5 BM lesions were associated with LM development after SRS. Chemotherapy and targeted therapy after LM were associated with better survival in patients with LM after SRS.

Summary: A single-arm Phase II study of 18 patients with leptomeningeal disease (LMD) receiving combined ipilimumab and nivolumab showed promising activity with acceptable safety profile. Larger multicenter clinical trials are needed to validate these results for the rare complication with a very poor prognosis.

Summary: Glioblastoma patients with leptomeningeal disease (LMD) are facing a grim prognosis, with untreated patients surviving only a few months. Treatment has shown to significantly improve overall survival, although there is no specific therapy that stands out in significantly improving survival in LMD over another. Additional studies are needed to establish a standard of care for LMD.

Summary: This study investigates the therapeutic efficacy of oncolytic Herpes simplex virus 1 (HSV1) on different stages of breast cancer leptomeningeal metastases growth. The results show that targeting the lag and intermediate phases of the disease can inhibit tumor growth, delay symptoms onset, and extend survival.

Summary: Compared with photon IFRT, proton craniospinal irradiation (pCSI) improved CNS progression-free survival (PFS) and overall survival (OS) for patients with non-small-cell lung cancer and breast cancer with leptomeningeal metastasis (LM), with no increase in serious treatment-related adverse events (TAEs).
