Tracking accuracy of T2-and diffusion-weighted magnetic resonance imaging for infusate distribution by convection-enhanced delivery Laboratory investigation

Object. Because convection-enhanced delivery relies on bulk flow of fluid in the interstitial spaces, MR imaging techniques that detect extracellular fluid and fluid movement may be useful for tracking convective drug distribution. To determine the tracking accuracy of T2-weighted and diffusion-weighted MR imaging sequences, the authors followed convective distribution of radiolabeled compounds using these imaging sequences in nonhuman primates. Methods. Three nonhuman primates underwent thalamic convective infusions (5 infusions) with (14)C-sucrose (MW 342 D) or (14)C-dextran (MW 70,000 D) during serial MR imaging (T2- and diffusion-weighted imaging). Imaging, histological, and autoradiographic findings were analyzed. Results. Real-time T2- and diffusion-weighted imaging clearly demonstrated the region of infusion, and serial images revealed progressive filling of the bilateral thalami during infusion. Imaging analysis for T2- and diffusion-weighted sequences revealed that the tissue volume of distribution (Vd) increased linearly with volume of infusion (Vi; R(2) = 0.94, R(2) = 0.91). Magnetic resonance imaging analysis demonstrated that the mean +/- SD Vd/Vi ratios for T2-weighted (3.6 +/- 0.5) and diffusion-weighted (3.3 +/- 0.4) imaging were similar (p = 0.5). While (14)C-sucrose and (14)C-dextran were homogeneously distributed over the infused region, autoradiographic analysis revealed that T2-weighted and diffusion-weighted imaging significantly underestimated the Vd of both (14)C-sucrose (mean differences 51.3% and 52.3%, respectively; p = 0.02) and (14)C-dextran (mean differences 49.3% and 59.6%; respectively, p = 0.001). Conclusions. Real-time T2- and diffusion-weighted MR imaging significantly underestimate tissue Vd during convection-enhanced delivery over a wide range of molecular sizes. Application of these imaging modalities may lead to inaccurate estimation of convective drug distribution. (DOI: 10.3171/2011.5.JNS11246)

Summary: This paper proposes a deep learning algorithm based on a Siamese Network to classify functional resting state networks (RSNs) derived from Independent Component Analysis (ICA) decomposition. The method out-performs traditional convolutional neural network (CNN) and template matching methods in identifying subject-specific RSNs, achieving high accuracy on holdout and outside data sets. It is also robust to scan-rescan variation and reveals altered functional connectivity in a group analysis of traumatic brain injury (TBI).

Segmented 3D Echo Planar Acquisition for Rapid Susceptibility-Weighted Imaging: Application to Microhemorrhage Detection in Traumatic Brain Injury

Summary: The objective of this study was to evaluate the effectiveness of an accelerated 3D segmented echo planar imaging susceptibility-weighted imaging (segEPI SWI) in detecting cerebral microhemorrhages (CMBs) in traumatic brain injury (TBI). The results showed that segEPI SWI was comparable to the traditional 3D gradient echo SWI (GRE SWI) in CMB counts and diffuse axonal injury (DAI) diagnosis. Therefore, segEPI SWI has the potential to replace GRE SWI, especially in time-critical situations.

Summary: Patients with typical CM1 symptoms usually undergo MRI, and a TH of more than 5 mm is diagnosed as CM1, a TH between 3-5 mm is diagnosed as borderline CM1, and a TH of less than 3 mm with an associated cervical syrinx is diagnosed as CM0. However, patients are usually not diagnosed with CM if MRI shows less than 3-5 mm of TH and no syrinx. Recent MRI morphometric analysis has detected anatomical abnormalities that explain these symptoms.

Audiovestibular Findings in a Cohort of Patients with Chiari Malformation Type I and Dizziness

Summary: This study aimed to describe the audiovestibular phenotype in patients with CM1 and dizziness symptoms. Vestibular abnormalities, especially functional balance impairment, were common in these patients. Neck pain was negatively associated with somatosensory analysis score. Although isolated peripheral vestibulopathy was rare, vestibular/balance assessment is important for identifying patients who may benefit from specialized medical care.

Time-varying SUVr reflects the dynamics of dopamine increases during methylphenidate challenges in humans

Summary: Combined PET imaging and mathematical simulations show that the rate of dopamine increase in the striatum is the basis for self-reports of drug high in response to oral and intravenous methylphenidate challenges. This study reveals that the speed of dopamine increases, in addition to its amplitude, plays a role in drug reward. The findings suggest that intravenous methylphenidate induces faster dopamine increases compared to oral methylphenidate, and the time-to-peak is associated with the intensity of the self-report of high.

CHD7 variants associated with hearing loss and enlargement of the vestibular aqueduct

Summary: Enlargement of the endolymphatic sac, duct, and vestibular aqueduct (EVA) is a common inner ear malformation in patients with hearing loss, and is associated with pathogenic variants in SLC26A4. However, in European-Caucasian populations, around 50% of EVA patients have no pathogenic alleles of SLC26A4. This study found that CHD7 variants can cause EVA and nonsyndromic hearing loss, and suggest that CHD7 should be included in DNA sequence analyses for EVA patients.

Developing Semiautomated Methods to Measure Pre- and Postoperative Syrinx Volumes

Summary: Neurosurgeons use MRI scans to evaluate the reduction in syrinx size after surgical treatment. Manual measurement of syrinx volume is time-consuming and prone to errors. Developing convenient semiautomated methods for volumetric analysis can improve the monitoring of syringomyelia. This study compared two semiautomated methods (SCAT and 3DQI) with a manual method (CAV) and found that all methods detected a decrease in syrinx volume after surgery. The semiautomated methods showed efficient and accurate results for clinical decision-making and research studies.
