Summary: This study evaluated the safety and effectiveness of using direct carotid puncture (DCP) for mechanical thrombectomy (MT) in stroke patients with anterior circulation acute ischemic stroke with large-vessel occlusion and prohibitive vascular access. The results showed that DCP was associated with higher recanalization rates, smaller infarct volumes, and improved functional outcomes compared to patients with failed transfemoral access, suggesting that DCP should be considered in this patient population.

It is critical to consider these technical limitations to ensure the safest care possible for children undergoing thrombectomy for acute stroke.

Summary: This study aimed to evaluate the impact of the hyperdense middle cerebral artery sign (HMCAS) on the outcome and technical challenges in patients undergoing endovascular thrombectomy (EVT). The results showed that HMCAS did not predict a poor outcome in patients treated with EVT. However, patients with HMCAS required longer procedure times and a higher number of thrombus passes.
