The influence of neuropathology on brain inflammation in human and experimental temporal lobe epilepsy

It is unclear to what extent neuropathological changes contribute to brain inflammation observed in temporal lobe epilepsy (TLE). Here, we compared cytokine levels between histopathologically-confirmed sclerotic hippocampi and histopathologically-confirmed normal hippocampi from TLE patients. We analyzed a similar cytokine panel in the hippocampi of amygdala-kindled rats and we evaluated neuropathological changes by immunohistochemistry. In TLE patients, cytokine levels were not significantly different between sclerotic and nonsclerotic hippocampi. Though kindling resulted in increased astrocyte activation, cytokine levels and microglia activation were unchanged. These results suggest that the chronic epileptic state in TLE can also occur in the absence of intracerebral inflammation. (C) 2014 Elsevier B.V. All rights reserved.

Bone mineral density and fractures in institutionalised children with epilepsy and intellectual disability

Summary: This study found that 67% of institutionalised children with epilepsy and intellectual disability had low bone mineral density, and 42% had a history of at least one fracture. Despite supplementation of calcium and vitamin D according to Dutch guidelines, the prevalence of bone disorders and fractures remains high in this population.

Economic evaluation of deep brain stimulation compared with vagus nerve stimulation and usual care for patients with refractory epilepsy: A lifetime decision analytic model

Summary: This study suggests that, given current limited evidence, VNS and DBS are potentially cost-effective treatment strategies compared to CAU for patients with refractory epilepsy. However, results for DBS were heavily impacted by assumptions made to extrapolate nonresponse from the original trial, indicating a need for further empirical research to reduce uncertainty.

Assessment of Trabecular Bone Score: a 7-year follow-up study in institutionalized adults with refractory epilepsy and intellectual disability

Summary: This longitudinal study aimed to assess trabecular bone scores (TBS) in institutionalized adults with refractory epilepsy and intellectual disability and study the association between TBS and incident fractures during seven years of follow-up. The results showed a high incidence of fractures and a significant decrease in TBS over the follow-up period, but TBS was not associated with incident fractures.

Economic evaluations of nonpharmacological treatments for drug-resistant epilepsy: A systematic review

Summary: This study systematically identified and evaluated published economic evaluations on nonpharmacological interventions for drug-resistant epilepsy. The researchers applied specific criteria to search for relevant studies and assessed their quality. The study found that epilepsy surgery, vagus nerve stimulation, and responsive neurostimulation are cost-effective treatments for adults, while the cost-effectiveness of epilepsy surgery in children and the use of ketogenic diet were inconclusive.
