A 21-year-old man with thalassemia intermedia presented with progressive decrease in vision and was found to have severe bilateral optic atrophy. Orbital and brain neuroimaging revealed massive diffuse expansion of the diploe of the craniofacial bones, narrowing of the optic canals, and expansion of hematopoietic tissue in the sellar region with compression of the optic chiasm. Although increased bone marrow is a well-known consequence of thalassemia, optic atrophy due to and such marked extramedullary expansion of hematopoietic tissue is a very rare phenomenon. Decompression of extramedullary hematopoietic tumors from thalassemia may be achieved with blood transfusions alone without the need for radiation or surgery. These findings suggest careful periodic ophthalmic examination of thalassemic patients.

Upper blepharoplasty: advanced techniques and adjunctive procedures

Parya Abdolalizadeh, Mohsen Bahmani Kashkouli, Vahid Khamesi, Nasser Karimi, Hossein Ghahvehchian, Leila Ghiasian

Summary: This study reviews the current concepts in preoperative considerations, surgical techniques, and adjuvant procedures of upper blepharoplasty, while also discussing the role of social media in this field. Careful examination of the midface and upper face is necessary to assess concomitant aging deformities. The volume preservation approach is now the mainstream technique used in upper blepharoplasty, and the incisions can be utilized to address other issues such as lacrimal gland prolapse, blepharoptosis, Asian medial epicanthoplasty, and eyebrow ptosis.

Summary: RADIANT is conducting a study on individuals with uncharacterized forms of diabetes and their family members. The study involves genomic, phenotypic, metabolomics, and metabolic assessments. The findings include new monogenic variants and different phenotypic clusters of atypical diabetes. The analyses will improve the identification of atypical diabetes and discover new mechanisms and biomarkers.

Impact of COVID-19 on the patient referral pattern and conversion rate in the university versus private facial plastic surgery centers

Parya Abdolalizadeh, Mohsen Bahmani Kashkouli, Soheila Jafarpour, Saeid Rezaei, Shaghayegh Ghanbari, Shadi Akbarian

Summary: This study compared the number of referrals and conversion rate between the pandemic and pre-pandemic period in two facial plastic surgery centers. The results showed that in the private center, the number of referrals declined by 7.7% and the conversion rate significantly decreased. In the university center, the number of referrals declined by 32%, but the conversion rate did not show a significant difference. Although the number of referrals recovered to almost the pre-pandemic level, the conversion rates remained lower.
