Background: Juvenile dermatomyositis is a rare childhood multisystem autoimmune disease involving primarily the skin and muscles, and it may lead to long-term disability. This study aimed to describe the clinical course of juvenile dermatomyositis and determine if any early clinical or laboratory features could predict outcome. Methods: Medical charts of patients aged <= 18 years and diagnosed with juvenile dermatomyositis (according to the criteria of Bohan and Peter) at the Pediatric Department, National Taiwan University Hospital, between 1989 and 2009 were reviewed. The endpoints for disease assessment were complete clinical response and complete clinical remission. Cox's proportional hazards model was fitted to identify important predictors of complete clinical remission. Results: A total of 39 patients with juvenile dermatomyositis were reviewed. Two-thirds were females, and the mean age at disease onset was 81.97 +/- 46.63 months. The most common initial presentations were Gottron's papule (82.1%) and muscle weakness (82.1%). After excluding one patient with an incomplete record, the remaining 31 patients who had muscle weakness were analyzed; among them, 22 (70.97%) achieved complete clinical response, but only six (19.4%) achieved complete clinical remission. Multivariate analysis showed that female sex, negative Gowers' sign at disease onset, and positive photosensitivity at disease onset were favorable factors to achieve complete clinical remission. Moreover, covariate-adjusted survival curves were drawn for making predictions of complete clinical remission. Only 13 (33.33%) patients were symptom free at the end of follow up, whereas the other 26 suffered from different kinds of complications. None of them developed malignancy, but two (5.13%) patients died during the follow-up period. Conclusion: Factors such as male sex and Gowers' sign were unlikely to favor the achievement of complete clinical remission in juvenile dermatomyositis. Certain complications cannot be avoided, and thus more effective treatments and monitoring strategies are needed for better control of juvenile dermatomyositis. Clinical features and outcomes of patients with chronic granulomatous disease in Taiwan

Ting-Sheng Lin, Jyh-Hong Lee, Li-Chieh Wang, Yao-Hsu Yang, Yu-Lung Lau, Wen- Lee, Yu-Tsan Lin, Bor-Luen Chiang, Hsin-Hui Yu

Summary: This study aimed to investigate the clinical features, genetic characteristics, treatment, and prognosis of patients with chronic granulomatous disease (CGD). Results showed that there were six patients with X-linked CGD and three patients with autosomal recessive CGD. Patients with X-linked CGD had earlier onset and diagnosis ages compared to those with autosomal recessive CGD. Prophylactic use of antibiotics, antifungal agents, and interferon-gamma might alleviate infection episodes in CGD patients. Rating: 7 out of 10.

Characterization of the biomarkers related to the clinical course and outcomes of juvenile dermatomyositis

Ting -Wei Lin, Ya-Chiao Hu, Bor-Luen Chiang

Summary: This study aimed to evaluate the clinical characteristics of children diagnosed with juvenile dermatomyositis (JDM) in Taiwan and identify biomarkers for predicting the disease course and outcomes. The study included 47 patients, with a mean age of 7.5 years at disease onset. The most common initial presentations were Gottron's sign, muscle weakness, and facial rash. The results identified factors associated with complete clinical remission and the chronic continuous course, highlighting the importance of aggressive treatment for the latter.

Summary: This retrospective cohort study aims to evaluate the real-world benefits of direct-acting antiviral (DAA)-induced sustained virologic response (SVR) on the occurrence and progression of esophageal varices (EV) in patients with hepatitis C virus (HCV)-related liver cirrhosis (LC). The study found that DAA-induced SVR was significantly associated with a lower risk of de novo EV occurrence and progression.

Summary: This study aimed to determine the risk factors for subsequent lupus nephritis (LN) development in patients with juvenile-onset systemic lupus erythematosus (jSLE). A retrospective cohort study found that out of 48 jSLE patients without initial LN, 20 developed subsequent LN. Persistently high levels of anti-double-stranded DNA antibody and erythrocyte sedimentation rate were identified as risk factors for subsequent LN in jSLE patients.

Evaluation of Infective Endocarditis in Children: A 19-Year Retrospective Study in Taiwan

Summary: This study compares clinical outcomes between pediatric patients with and without heart disease diagnosed with infective endocarditis (IE), and determines the risk of in-hospital death. The results show that patients with heart disease are more prone to streptococcal infections and cardiac complications, but have a lower mortality rate. Platelet count can serve as a risk factor for in-hospital mortality in pediatric patients with IE.

Perinatal Characteristics and the Sensitization to Cow Milk, Egg Whites and Wheat in Children up to 3 Years of Age

Hsin-Yu Chang, Zon-Min Lee, Ling-Sai Chang, Wei-Ling Feng, Yao-Hsu Yang, Mei-Chen Ou-Yang

Summary: Food sensitization in early life can identify children at risk of developing allergic diseases. This study investigated sensitization to cow milk, egg whites, and wheat. Data from the Chang Gung Research Database was used to conduct a retrospective survey. The study found that boys were more likely to have a positive immunoglobulin E (IgE) reaction to cow milk and egg whites, and early-life sensitization to egg whites and wheat was associated with increased birth weight and length. A multivariate analysis revealed that egg white IgE positivity was linked to higher levels of total IgE, and younger age and higher birth weight and length were associated with food sensitization, particularly to egg whites and wheat.

Recurrence of pediatric intussusception: A nationwide population-based descriptive study in Taiwan

Summary: This study investigated the risk factors associated with recurrent intussusception in children aged ≤ 3 years. Male patients had a higher risk of recurrent intussusception, and higher birth weight may increase the risk. Surgical treatment and delayed onset of intussusception were associated with a reduced risk. It is suggested to closely follow up early infancy patients who received non-surgical treatment for potential recurrence.

Summary: The Chiehyuan herbal oral protection solution (GB-2) is a traditional Chinese medicine commonly used in Taiwan to combat SARS-CoV-2. This study found that GB-2 can reduce the expression of ACE2 and TMPRSS2 in oral mucosal cells. It also showed that GB-2 can decrease the entry efficiency of various variants of SARS-CoV-2 and block the interaction between ACE2 and the RBD with specific mutations. These findings suggest that GB-2 may be a potential candidate for further research as a preventive measure against SARS-CoV-2.
