Summary: This article discusses the clinical role and scope of functional radionuclide imaging with 99mTc-pertechnetate in diagnosing testicular torsion in patients with acute hemiscrotum. The technique, characteristic findings, and differentiation from other conditions are described. SPECT imaging and hybrid SPECT/CT are shown to improve the accuracy of diagnosis. Ultrasonographic and color Doppler findings are also discussed. Several case examples demonstrate the clinical benefit of combining functional and structural imaging for better testicular imaging.

Summary: This study examines and compares the testicular salvage surgery rates for pediatric testicular torsion before and after the implementation of national quality metrics. The results show that the salvage surgery rate increased by 12.5% since the implementation of quality metrics. Patient factors such as age, race, distance to hospital, household income, insurance status, and use of ultrasound were similar between the two groups.

Summary: This study evaluated the association between climatic factors and the testicular hemodynamics and thermoregulatory responses of hair sheep rams raised in a humid tropical climate. The results showed that testicular temperature and blood flow were higher during the rainy season, and there was a lower correlation with environmental factors.

Summary: This study investigates the long-term effects of surgery for testicular torsion (TT) on endocrine and exocrine testicular function, oxidative stress in semen, and erectile function. The results show that TT does not significantly affect these functions in the long term. Preservation of the testis is associated with a higher round cell count and follicle stimulating hormone (FSH) compared to orchiectomy.

Summary: In some cases of testicular torsion, Color Doppler may show normal or increased blood flow signals in the testis. We should further observe the morphology and position of the testes and epididymides, the echo of the testicular parenchyma, and, especially evaluate the whirlpool sign in the spermatic cord, to avoid missing testicular torsion with blood flow signals.

Summary: This study proposed a modified surgical technique for immediate intravaginal prosthesis implantation in patients undergoing orchiectomy due to testicular torsion, and evaluated the wound healing process and patient satisfaction. The results showed that the modified intravaginal surgical technique was safe and effective, with no prosthesis extrusion and high patient satisfaction rate.

Summary: The commonly taught notion that intratesticular lesions are always malignant and extratesticular scrotal lesions are always benign is oversimplified and overlooks the importance of extratesticular scrotal masses and their diagnosis. This article presents a framework for differential diagnosis of extratesticular scrotal masses, discusses the range of pathologic conditions that can be encountered, and familiarizes radiologists with the sonographic appearances of these lesions. It also highlights the importance of recognizing findings that warrant additional imaging or surgery, and the potential use of scrotal MRI when ultrasound is inconclusive.

Summary: This study aimed to investigate whether prior testicular torsion is associated with testicular function in young men from the general population. The results showed no differences in semen parameters between men who had experienced testicular torsion and controls. However, subtle alterations in reproductive hormones were observed, with higher FSH and lower inhibin B levels in men with prior testicular torsion. Follow-up studies are needed to assess any long-term consequences for fertility.
