Summary: Preterm premature rupture of membranes (PPROM) leading to preterm birth has significant implications for neonatal and maternal health. This systematic review examined existing data on biomarkers that can predict the duration of pregnancy following PPROM. Several biomarkers, including IL-6, IL-8, CRP, IL1RA, and others, showed a positive predictive value for timing of delivery. Proinflammatory biomarkers, particularly IL-6 or CRP, were found to be the most correlated with delivery timing, regardless of intrauterine infection.

Summary: Preterm premature rupture of membranes is associated with an increase in matrix metallopeptidases (MMPs) and a decrease in their inhibitors, tissue inhibitor metallopeptidases (TIMPs). Granulocyte-macrophage colony-stimulating factor (GM-CSF) has been shown to weaken fetal membranes. Treating pregnant mice with GM-CSF led to increased MMPs:TIMPs, resulting in membrane rupture and preterm birth.

Summary: This secondary analysis of the Twin Birth Study demonstrated that the planned delivery mode (CD or VD) in twin pregnancies with the first twin in cephalic presentation may have different effects on perinatal outcomes depending on gestational age at delivery. For pregnancies delivered at 32-37 weeks, planned VD appeared to be favorable, while from around 37 weeks onwards, planned CD might be safer. Evaluating the risks and benefits of each delivery mode is crucial to optimize perinatal outcomes in twin pregnancies.
