Summary: Severe endometriosis can cause dense adhesions between the posterior cervical peritoneum and the anterior sigmoid or rectum, leading to obliteration of the cul-de-sac and distortion of normal anatomic landmarks. Surgeons should focus on not only avoiding ureteral and rectal injuries, but also preserving the hypogastric nerves to prevent complications. This article reports on the anatomical highlights and surgical steps of laparoscopic hysterectomy for posterior cul-de-sac obliteration with the nerve-sparing technique.

Summary: This systematic review and meta-analysis evaluated the diagnostic accuracy of the sliding sign on transvaginal ultrasound in detecting pouch of Douglas obliteration and bowel involvement in patients with suspected endometriosis, using laparoscopy as the reference standard. The results showed that the sliding sign had good diagnostic performance for predicting pouch of Douglas obliteration and bowel involvement.

Summary: The study aimed to demonstrate the coexistence of endometriomas with peritoneal and deep infiltrating endometriosis, as well as with adhesions. It was a randomized retrospective study including 1054 patients treated for endometrioma at Academic Hospital Cologne Weyertal from January 2014 to October 2019. The results showed a high prevalence of peritoneal and deep infiltrating endometriosis in patients with endometriomas, with the need for surgical treatment to be cautious due to the potential presence of adhesions and complications.

Summary: Posterior Cul-de-sac rupture is a rare delivery complication that poses a diagnostic challenge to obstetricians. It can occur without any predisposing risk factors and may be misdiagnosed due to vague and unspecific symptoms. Therefore, a high level of suspicion should be maintained for concealed vaginal wall rupture when sudden severe pain during labor cannot be otherwise explained.

Summary: This article reports a case of cervical cerclage with intrauterine balloon tamponade (IBT) for placenta previa with cervical dilation. During the operation, cervical dilation and prolapse of the intrauterine balloon were observed, leading to the use of absorbable sutures for cervical cerclage and blood transfusion. The results suggest that cervical cerclage with IBT is effective in preventing intrauterine balloon prolapse in cases with cervical dilation.
