Monotherapy vs combination treatments of different complexity: a meta-analysis of blood pressure lowering randomized outcome trials

Summary: Using drug combinations for hypertension treatment is recommended for most patients as they show better blood pressure-lowering effects compared to monotherapy. Randomized clinical trials indicate that combination treatments reduce fatal and nonfatal outcomes compared to simpler treatment regimens, however, the presence of treatment-related adverse events may affect the overall benefit of combination therapy.

Summary: This study evaluated the differential outcomes of different hypertension phenotypes based on clinic and ambulatory blood pressure measurements. It found that isolated systolic hypertension (ISH) based on clinic blood pressure was associated with a higher incidence of cardiovascular events compared to isolated diastolic hypertension (IDH) and systolic/diastolic hypertension (SDH). Integration of clinic and ambulatory blood pressure measurements also increased the outcome incidence for ISH.

Summary: The meta-analysis of nine studies showed that patients undergoing renal sympathetic denervation (RDN) had significantly lower left ventricular volumes and higher ejection fraction compared to the control group. However, further large, randomized studies are needed to elucidate the role of RDN in cardiac remodeling in hypertension, heart failure, and other clinical settings.

Mediterranean diet and blood pressure reduction in adults with and without hypertension: A systematic review and meta-analysis of randomized controlled trials

Summary: This study comprehensively assessed the effects of the Mediterranean diet on blood pressure levels through a meta-analysis of 35 randomized controlled trials. The findings suggest that the Mediterranean diet may lead to a small but significant reduction in blood pressure compared to usual and other active intervention diets, with greater effects observed in trials with higher baseline blood pressure and longer follow-up durations.

Summary: The study aimed to evaluate the impact of waist circumference on left-ventricular (LV) structural and functional alterations in hypertensive males and females. Results showed that waist circumference had a more pronounced effect on LV alterations in female hypertensive patients compared to males, suggesting routine measurement of waist circumference as an additional predictor of cardiac phenotype in women.

Summary: The study revealed that 41.4% of pregnant women in Athens were smokers, with 52.4% of them quitting smoking after finding out about their pregnancy. Factors such as young age, multiparity, low education level, and having a smoking partner were found to predispose pregnant women to smoking.
