Summary: This study focuses on the differential expression and localization of cell-cell contact proteins E-cadherin and N-cadherin in primary liver cancer and liver metastases of ductal adenocarcinoma of the pancreas. The study demonstrates that N-cadherin positivity can distinguish intrahepatic cholangiocarcinoma from liver metastases of ductal adenocarcinoma of the pancreas, improving the accuracy of the diagnosis. The findings suggest that N-cadherin can be a helpful marker for differential diagnosis between these two types of cancers.

Summary: Pancreatic adenocarcinoma is a leading cause of cancer-related morbidity and mortality with most patients presenting with advanced disease. Preoperative biliary drainage (PBD) has traditionally been used to improve surgical outcomes, but recent data suggests potential increased complications with systematic PBD. Indications for PBD include cholangitis, delayed surgery, and jaundice relief in patients planned to receive neoadjuvant therapy. Endoscopic retrograde cholangiopancreatography is the preferred method for PBD, with self-expanding metal stents shown to be cost-effective and safe compared to plastic stents.

Summary: The study aimed to investigate the correlation between MRI features and biomarkers, as well as prognostic factors for RFS and OS in cHCC-CCA patients. Results showed that cHCC-CCA imaging features were not correlated with AFP or CA19-9 levels. Age, CA19-9 > 37 U/ml, arterial phase peritumoral enhancement, and delayed enhancement were identified as independent predictors of poor RFS. Arterial phase peripheral enhancement was found to be an independent predictor of poor OS.

Summary: Multiple studies have shown that recurrent genetic and epigenetic abnormalities are associated with worse prognosis in neuroendocrine tumors (NETs). However, most of these studies are small and only conducted univariate analysis. This review and meta-analysis focus on NETs in the gastrointestinal tract, liver, biliary tract, and pancreas, and found specific genetic and epigenetic alterations that are associated with worse survival outcomes. It also emphasizes the importance of considering NETs from different anatomical locations as distinct biological entities with different molecular prognostic markers.

Biomarkers obtained from patients' serum or tumor tissue could help guide therapy and identify those at higher risk of recurrence. Genetic aberrations in cholangiocarcinoma have also been linked with improved response to targeted therapies. This review provides an overview of prognostic and predictive biomarkers in cholangiocarcinoma.

Summary: This systematic review and meta-analysis evaluated the diagnostic performance of percutaneous transluminal forceps biopsy (PTFB) for differentiating malignant from benign biliary strictures. The results showed that PTFB had high sensitivity and specificity for diagnosing malignant biliary strictures. It had higher sensitivity for intrinsic biliary strictures compared to extrinsic ones. The rate of complications was 10.3%, with a 3.1% rate of major complications.

Summary: Although nodal metastasis is an important prognostic factor in ampullary adenocarcinoma, the prognostic significance of extranodal extension (ENE) is not well defined. This study investigated the presence of nodal metastasis with ENE in 279 surgically resected patients and compared it with other clinicopathological factors, including overall survival and recurrence-free survival. The results showed that ENE was associated with poorly differentiated tumors and had worse overall survival and recurrence-free survival rates. Evaluation of ENE can provide additional survival information for patients with surgically resected ampullary carcinoma.

Intracystic pancreatic intraepithelial neoplasia (PanIN) and driver mutations were rare, suggesting the need for preoperative diagnosis and long-term follow-up of ACT.
