Summary: NB-UVB and UVA1 have been effective for treating AD since the 1980s, but their clinical use in the biologic era needs further evaluation. 145 patients undergoing phototherapy achieved a median EASI score of 9.90 with UVA1 and 13.70 with NB-UVB. The rates of patients achieving IGA 0/1 were 33% (UVA1) and 28% (NB-UVB) for at least 6 months.

Summary: Pediatric patients with liver disease commonly experience itching, which can significantly affect their quality of life. The cause of itching in liver disease is not fully known and may be caused by many factors. Treating this symptom can be difficult and may require surgical interventions in certain cases. Understanding the cause of itching may help identify better treatment options.

Summary: Pruritus associated with chronic liver disease has a complex and not fully understood pathophysiology. For patients whose quality of life is affected by severe itching, liver transplantation may be considered if medical therapy fails to provide relief. Further research is needed to fully understand the pathophysiology of pruritus in chronic liver disease, which would help in developing more effective treatments for this condition.

Summary: This study evaluated the efficacy of therapies for cholestatic pruritus (CP), finding that existing treatments and newer approaches, such as noninvasive interventions and the ileal bile acid transporter inhibitor (IBATi), can effectively alleviate CP symptoms. However, further research is needed to determine the full extent of IBATi efficacy and its potential use in treating other cholestatic liver diseases (CLDs).

Summary: The study found that DEFB1 gene polymorphisms are associated with the risk of vitiligo development, the extent of the disease, and the response to NB-UVB treatment. The AA genotype of (-20G/A) is protective against vitiligo development, while the GG genotype and G allele of (-44 C/G) increase the risk. Different DEFB1 gene polymorphisms may modify the risk of vitiligo development, disease extent, and response to NB-UVB phototherapy.

Summary: This study investigated whether patients with 1-2 cm appendiceal neuroendocrine tumors (NETs) need to undergo right-sided hemicolectomy, and found that right-sided hemicolectomy is not necessary after appendectomy for appendiceal NETs. Furthermore, it was found that regional lymph node metastases of appendiceal NETs are clinically irrelevant and additional postoperative exclusion of metastases and histopathological evaluation of risk factors is not supported. These findings should inform the development of treatment guidelines.

Summary: Spontaneous portosystemic shunts are often overlooked as a cause of hepatic encephalopathy associated with cirrhosis. Summary: This study aimed to identify metabolites associated with short-term death in patients with acutely decompensated cirrhosis and design metabolomic prognostic models. Three prognostic metabolites strongly associated with death were selected and two models were built, which were found to be more accurate in predicting death within 7, 14, and 28 days compared to traditional scoring systems.

Summary: Endoscopy is an indispensable tool in diagnosing and managing liver disease and its complications. It has become an alternative route for interventions and is increasingly being used as a first-line choice. Endoscopic ultrasound is valuable for evaluating liver parenchyma, lesions, and surrounding tissues, as well as guiding portal pressure measurement and managing complications of portal hypertension. Hepatologists should be aware of the full spectrum of diagnostic and therapeutic tools available in this field. Future directions for endoscopy in hepatology are also discussed in this comprehensive review.

Summary: Variceal bleeding is a serious complication of portal hypertension in cirrhosis patients. When combined with acute-on-chronic liver failure (ACLF), variceal bleeding poses a higher risk of treatment failure and short-term mortality. Treating the underlying causes and reducing portal pressure can improve outcomes in patients with acute decompensation or ACLF. Transjugular intrahepatic portosystemic shunts (TIPSs), especially in preemptive cases, have been found to effectively control bleeding, prevent rebleeding, and decrease short-term mortality. Therefore, TIPS placement should be considered as a treatment option for ACLF patients with variceal bleeding.

Summary: Hepatocellular carcinoma (HCC) is a leading cause of cancer death, and the medical treatment options are limited. The multikinase inhibitor sorafenib has been widely used for systemic therapy in advanced HCC. This study shows that sorafenib affects the levels of polyunsaturated fatty acids (PUFA)-derived epoxygenated metabolites, indicating that specific supplementation with DHA could increase levels of the epoxy compound 19,20-EDP with potential anti-tumor activity in HCC patients receiving sorafenib therapy.

Summary: Abnormal polyol metabolism, primarily associated with diabetes, is also involved in phosphomannomutase 2 congenital disorder of glycosylation (PMM2-CDG). A potential therapy for PMM2-CDG, epalrestat, inhibits aldose reductase (AR). This study reveals that PMM2 deficiency affects intracellular glucose flux, leading to increased polyol production, and epalrestat treatment improves the clinical and biochemical characteristics of PMM2-CDG individuals.
