Cholestasis-induced pruritus treated with ultraviolet B phototherapy: An observational case series study

Background & Aims: Pruritus is a disabling complication of cholestatic liver disorders. Its management remains challenging. Ultraviolet B (UVB) phototherapy has been successfully used to treat pruritus in other indications. Methods: This is an observational case series. The study population consists of 13 patients (10 females, mean age 52 years) with pruritus due to different cholestatic liver disorders: PBC (n = 4), PSC (n = 2), drug-induced (n = 3) and persistent cholestasis after liver transplantation (LT) (n = 4). Serum alkaline phosphatase levels were: 686 +/- 363 mu/L and serum bile acids levels: 147 +/- 15 mu mol/L. In all patients, conventional medical treatment had failed to control pruritus. Perception of pruritus was recorded by the visual analogue scale (VAS). Results: The mean follow-up was 3 years. Ten patients (77%) had more than 60% reduction in perceived pruritus of which 4 had more than an 80% reduction. Median [25-75% percentiles] VAS score before and after treatment decreased from 8.0 [8.0-10] to 2.0 [1.5-2.1] (p <0.001). The mean number of irradiations required to obtain this effect was 26 +/- 17 (average duration of phototherapy: 8 weeks). No significant changes in cholestatic serum markers were observed. Four patients (30%) needed an additional phototherapy course because of recurrent pruritus and in all of them again a marked improvement of pruritus was observed. The therapy was well tolerated, except in two patients who developed, during retreatment, pronounced erythema in one case and paresthesia in the other case. Conclusions: UVB phototherapy appears to be a promising and well tolerated treatment also for cholestasis-associated pruritus. (C) 2012 European Association for the Study of the Liver. Summary: This study investigated whether patients with 1-2 cm appendiceal neuroendocrine tumors (NETs) need to undergo right-sided hemicolectomy, and found that right-sided hemicolectomy is not necessary after appendectomy for appendiceal NETs. Furthermore, it was found that regional lymph node metastases of appendiceal NETs are clinically irrelevant and additional postoperative exclusion of metastases and histopathological evaluation of risk factors is not supported. These findings should inform the development of treatment guidelines.

Summary: Spontaneous portosystemic shunts are often overlooked as a cause of hepatic encephalopathy associated with cirrhosis. They are considered as radiological biomarkers of clinically significant portal hypertension and have a clinical impact on complications related to portal hypertension. Interventional radiology embolization is a direct method to target these shunts.

Summary: This study aimed to identify metabolites associated with short-term death in patients with acutely decompensated cirrhosis and design metabolomic prognostic models. Three prognostic metabolites strongly associated with death were selected and two models were built, which were found to be more accurate in predicting death within 7, 14, and 28 days compared to traditional scoring systems.

Summary: This study aimed to assess the prevalence of alcohol-related hepatitis (AH) in patients with variceal bleeding and compare the clinical outcomes of AH patients with other alcohol-related liver disease (ALD) phenotypes and viral cirrhosis. The results showed that AH patients had more complications and abstinent ALD patients had higher survival rates compared to other groups.

Summary: Endoscopy is an indispensable tool in diagnosing and managing liver disease and its complications. It has become an alternative route for interventions and is increasingly being used as a first-line choice. Endoscopic ultrasound is valuable for evaluating liver parenchyma, lesions, and surrounding tissues, as well as guiding portal pressure measurement and managing complications of portal hypertension. Hepatologists should be aware of the full spectrum of diagnostic and therapeutic tools available in this field. Future directions for endoscopy in hepatology are also discussed in this comprehensive review.

To TIPS or Not to TIPS in High Risk of Variceal Rebleeding and Acute-on-Chronic Liver Failure

Summary: Variceal bleeding is a serious complication of portal hypertension in cirrhosis patients. When combined with acute-on-chronic liver failure (ACLF), variceal bleeding poses a higher risk of treatment failure and short-term mortality. Treating the underlying causes and reducing portal pressure can improve outcomes in patients with acute decompensation or ACLF. Transjugular intrahepatic portosystemic shunts (TIPSs), especially in preemptive cases, have been found to effectively control bleeding, prevent rebleeding, and decrease short-term mortality. Therefore, TIPS placement should be considered as a treatment option for ACLF patients with variceal bleeding.

Summary: Hepatocellular carcinoma (HCC) is a leading cause of cancer death, and the medical treatment options are limited. The multikinase inhibitor sorafenib has been widely used for systemic therapy in advanced HCC. This study shows that sorafenib affects the levels of polyunsaturated fatty acids (PUFA)-derived epoxygenated metabolites, indicating that specific supplementation with DHA could increase levels of the epoxy compound 19,20-EDP with potential anti-tumor activity in HCC patients receiving sorafenib therapy.

Summary: Abnormal polyol metabolism, primarily associated with diabetes, is also involved in phosphomannomutase 2 congenital disorder of glycosylation (PMM2-CDG). A potential therapy for PMM2-CDG, epalrestat, inhibits aldose reductase (AR). This study reveals that PMM2 deficiency affects intracellular glucose flux, leading to increased polyol production, and epalrestat treatment improves the clinical and biochemical characteristics of PMM2-CDG individuals.
