Summary: The aim of this study was to assess the association between graft-to-recipient weight ratio (GRWR) and hepatocellular carcinoma (HCC) recurrence in adult-to-adult living donor liver transplantation (LDLT). Three studies were identified, involving a total of 782 patients. The results showed that GRWR < 0.8% was associated with lower survival rates and higher tumor recurrence rates in HCC recipients, particularly for patients with HCC outside the established criteria. Considering individual GRWR as a contributor to HCC recurrence in LDLT patients may be beneficial.

Summary: Liver transplantation provides the best survival benefit for hepatocellular carcinoma (HCC) patients who cannot undergo resection. The Milan criteria have been developed to select suitable candidates for transplantation, aiming to improve survival and reduce the risk of HCC recurrence. However, up to 20% of cases experience HCC recurrence after transplantation, posing a significant challenge due to poor prognosis. Various extended criteria have been proposed to accommodate the increasing demand for organs and the associated higher risk of recurrence. Radiologists need to be aware of the possibility of HCC recurrence involving multiple organs after transplantation. Understanding the location and radiologic appearance of recurrent HCC is crucial for choosing the most appropriate therapy.

Summary: The study evaluated the prognostic impact of the ADV score for predicting HCC recurrence and patient survival after LDLT in 843 HCC patients. The ADV score showed a significant correlation with TR and OS, providing further prognostic stratification beyond existing criteria. Compared to pre-existing selection criteria, the ADV score with a cutoff of 5log exhibited the highest prognostic contrast for TR and OS, making it a valuable prognostic tool for posttransplant prognosis in HCC patients.

Adult and pediatric recipients of NDD liver transplants were more frequently female and diagnosed with biliary atresia. There were no observed racial inequities in the distribution of NDD grafts at different centers for adult or pediatric patients.

Summary: The aim of this study was to evaluate the risk factors for malpractice litigation in patients with bile duct injury (BDI) referred to a tertiary care center. The results showed that patients in the late-referral group were more likely to have undergone surgical and endoscopic repair before referral. Prompt referral to experienced tertiary care centers without any attempt at repair may reduce the risk of litigation.

Summary: The individual prediction of post-liver transplantation hepatocellular carcinoma (HCC) recurrence risk is important. By analyzing the data of 4981 HCC patients undergoing transplantation, a prediction score was developed to accurately discriminate the risk of HCC recurrence. The score was externally validated in another 1160 patients, showing consistent results in risk discrimination.

Summary: This study aimed to evaluate the magnesium content in human cirrhotic liver and its correlation with serum AST levels, expression of hepatocellular injury, and MELDNa prognostic score. The results showed that patients with cirrhosis had lower hepatic magnesium content and higher percentage of TRPM7-positive hepatocytes. The increase in hepatic magnesium content and TRPM7 expression were associated with worsening of MELDNa score. These findings suggest that severity of hepatocyte injury and prognosis in cirrhosis are closely related to magnesium depletion and overexpression of TRPM7.

Donor GGT is found to be an independent predictor of graft loss and its inclusion in traditional prediction models improves the accuracy. A collaborative effort by the ERAS4OLT.org partnership and specialists from around the world resulted in 80 final recommendations covering various aspects of enhanced recovery for liver transplantation. These guidelines provide a comprehensive overview and can guide the development of enhanced recovery programs globally.

Summary: This paper reviews the recent developments in liver transplantation (LT) for unresectable colorectal liver metastases (CRLM) and cholangiocellular carcinoma (CCA), and suggests that integration of LT with (neo)-adjuvant chemotherapy, radiotherapy, or locoregional treatments can effectively cure selected patients. However, further advances in diagnosis, treatment protocols, and donor-to-recipient matching are needed to better define the oncological indications for transplantation. Prospective, multicenter trials are still required to study these advances and their impact on outcomes.
