Recurrence of hepatocellular cancer after liver transplantation: The role of primary resection and salvage transplantation in East and West

Background & Aims: Greater tumor aggressiveness and different management modalities of hepatocellular cancer (HCC) before liver transplantation (LT) may explain the higher recurrence rates reported in Asia. This study investigates the prognostic factors for HCC recurrence in a Western and an Eastern HCC patient cohort in order to analyze the respective roles of tumor- and management-related factors on the incidence of post-LT HCC recurrence. Methods: Data of 273 HCC patients, transplanted during the period January 1999-March 2009, were obtained from the Rome Inter-University Liver Transplant Consortium (n = 157) and Hong Kong University (n = 116) databases. Median follow-up was 4.3 years (range: 0.2-12). Recurrence rate and multivariate logistic regression analysis was performed on the entire population and on Milan criteria-in (MC-in) patients. Results: Multivariate analysis on the entire population identified four independent risk factors for post-LT HCC recurrence: microvascular invasion (odds ratio, OR = 4.88; p = 0.001), poor tumor grading (OR = 6.86; p = 0.002), diameter of the largest tumor (OR = 4.72; p = 0.05), and previous liver resection (LR) (OR = 3.34; p = 0.04). After removal of LR, only tumor-related variables were independent risk factors for recurrence. When only MC-in patients were analyzed, no difference was observed between the two cohorts in terms of recurrence rate after LR patient removal. Conclusions: LR followed by salvage for HCC recurrence LT represents the main reason for a higher HCC recurrence rate in the Hong Kong patients, but not LR followed by salvage for liver failure LT in the Roman group. This approach towards HCC before LT may not be universally applicable. The precise patient background must be taken into account in order to identify the best pre-LT strategy. Bile duct injury following cholecystectomy: delayed referral to a tertiary care center is strongly associated with malpractice litigation

Summary: The aim of this study was to evaluate the risk factors for malpractice litigation in patients with bile duct injury (BDI) referred to a tertiary care center. The results showed that patients in the late-referral group were more likely to have undergone surgical and endoscopic repair before referral. Prompt referral to experienced tertiary care centers without any attempt at repair may reduce the risk of litigation.

Summary: The individual prediction of post-liver transplantation hepatocellular carcinoma (HCC) recurrence risk is important. By analyzing the data of 4981 HCC patients undergoing transplantation, a prediction score was developed to accurately discriminate the risk of HCC recurrence. The score was externally validated in another 1160 patients, showing consistent results in risk discrimination.

Summary: This study aimed to evaluate the magnesium content in human cirrhotic liver and its correlation with serum AST levels, expression of hepatocellular injury, and MELDNa prognostic score. The results showed that patients with cirrhosis had lower hepatic magnesium content and higher percentage of TRPM7-positive hepatocytes. The increase in hepatic magnesium content and TRPM7 expression were associated with worsening of MELDNa score. These findings suggest that severity of hepatocyte injury and prognosis in cirrhosis are closely related to magnesium depletion and overexpression of TRPM7.

Summary: This study aims to demonstrate the adverse effect of donor gamma-glutamyl transferase (GGT) on graft loss in a large European liver transplant cohort. Donor GGT is found to be an independent predictor of graft loss and its inclusion in traditional prediction models improves the accuracy. However, the exact mechanisms linking GGT and poor transplant outcomes need further investigation.

Enhanced recovery for liver transplantation: recommendations from the 2022 International Liver Transplantation Society consensus conference

Summary: There is significant controversy surrounding enhanced recovery for liver-transplant recipients, and this review aimed to summarize current knowledge, identify key components, and create internationally accepted guidelines. A collaborative effort by the ERAS4OLT.org partnership and specialists from around the world resulted in 80 final recommendations covering various aspects of enhanced recovery for liver transplantation. These guidelines provide a comprehensive overview and can guide the development of enhanced recovery programs globally.

Summary: This paper reviews the recent developments in liver transplantation (LT) for unresectable colorectal liver metastases (CRLM) and cholangiocellular carcinoma (CCA), and suggests that integration of LT with (neo)-adjuvant chemotherapy, radiotherapy, or locoregional treatments can effectively cure selected patients. However, further advances in diagnosis, treatment protocols, and donor-to-recipient matching are needed to better define the oncological indications for transplantation. Prospective, multicenter trials are still required to study these advances and their impact on outcomes.
