Paroxysmal nocturnal hemoglobinuria in Budd-Chiari Syndrome: Findings from a cohort study

Background/Aims: A well recognized cause of Budd-Chiari syndrome (BCS) is paroxysmal nocturnal hemoglobinuria (PNH). PNH is an acquired disorder of hematopoietic stem cells, characterized by intravascular hemolysis and venous thrombosis. Testing for this hematological disorder should be considered in all BCS patients. Methods: Using data from the EN-Vie study, a multi-center study of 163 patients with BCS, we investigated the relationship between BCS and PNH in 15 patients with combined disease and compared the results to 62 BCS patients in whom PNH was excluded. Results: Median follow-up for the study group (n = 77) was 20 months (range 0-44 months). BCS patients with PNH presented with a significantly higher percentage of additional splanchnic vein thrombosis (SVT) as compared to BCS patients without PNH (47%, vs. 10%, p = 0.002). During follow-up, type and frequency of interventions for BCS was similar between both groups. Six patients with BCS and PNH were successfully treated with a transjugular intrahepatic portosystemic shunt (TIPS). Of 15 patients with PNH, six underwent allogenic stem cell transplantation after diagnosis of BCS. PNH was successfully cured in five cases. There was no significant difference in survival between BCS patients with and without PNH. Conclusions: This study shows that despite a higher frequency of additional SVT, short-term prognosis of BCS patients with PNH does not differ from BCS patients without PNH. Treatment with TIPS can be safely performed in patients with PNH. Stem cell transplantation appears to be a feasible treatment option for PNH in BCS patients. However, the referral rate is very low, high in late-stage diagnoses, and low in new diagnoses, indicating a need for early detection strategies in primary care for HCV infection.

Definition of a therapeutic range for predicting long-term infliximab response in patients with inflammatory bowel disease

Summary: The aim of this study was to determine the predictive potential of monitoring infliximab plasma levels for sustained clinical response and evaluate its usefulness in improving treatment efficacy and symptom control in patients with inflammatory bowel disease. The results confirmed the predictive potential of monitoring infliximab plasma levels as a strategy to evaluate sustained long-term clinical response. The study highlights the importance of introducing this monitoring into routine clinical practice to identify non-responders, optimize treatment, prevent relapse, and improve long-term therapy maintenance.

Summary: The Spanish Society of Digestive Pathology (SEPD), the Spanish Association for the Study of the Liver (AEEH), the Spanish Society of Infections and Clinical Microbiology (SEIMC) and its Viral Hepatitis Study Group (GEHEP), with the endorsement of the Alliance for the Elimination of Viral Hepatitis in Spain (AEHVE), have developed a document for comprehensive diagnosis of viral hepatitis (B, C, and D) from a single blood sample. They propose an algorithm to analyze the rest of the viruses, including viral load, when a positive result for viral hepatitis or HIV is found in the blood sample. They also recommend ruling out previous hepatitis A virus infection and determining the HIV serology. Automated alerts and epidemiological monitoring should be initiated in case of a positive result for any of the viruses.

Summary: The aim of this study was to describe the incidence of recurrent thrombosis (RT) in patients with non-cirrhotic splanchnic vein thrombosis (NC-SVT) and identify its risk factors. The study found that the risk of RT in NC-SVT patients without an indication for long-term anticoagulation is not low. Furthermore, factor VIII level may be a key indicator for predicting RT.

A Novel Insertion in the Hepatitis B Virus Surface Protein Leading to Hyperglycosylation Causes Diagnostic and Immune Escape

Summary: Chronic hepatitis B virus (HBV) infection is a global health threat. Mutations in the surface antigen of HBV (HBsAg) may alter its antigenicity, infectivity, and transmissibility. A patient positive for HBV DNA and detectable but low-level HBsAg in parallel with anti-HBs suggested the presence of immune and/or diagnostic escape variants. Secreted HBsAg was also subjected to four widely used, state-of-the-art diagnostic assays, which all failed to detect the hyperglycosylated insertion variant. Additionally, the recognition of mutant HBsAg by vaccine- and natural infection-induced anti-HBs antibodies was severely impaired.
