Summary: A comparison study of EUS-GBD and LC for acute cholecystitis found similar outcomes in terms of technical and clinical success rates, lengths of hospital stay, 30-day adverse events, and mortality rates. Additionally, the rates of recurrent biliary events, reinterventions, and unplanned readmissions within 1 year were also similar between the two procedures. These findings support EUS-GBD as a viable alternative to LC for patients who may not be suitable for definitive cholecystectomy.

Summary: This systematic review compares percutaneous transhepatic gallbladder biliary drainage (PTGBD) with emergency cholecystectomy (EC) as definitive treatments for high-risk patients with acute cholecystitis (AC). The analysis suggests that PTGBD is not associated with lower morbidity compared to EC. EC is associated with lower postoperative mortality, reduced risk of hospital readmission for biliary complications, and shorter hospital stay. Therefore, EC may be the preferred treatment for high-risk patients.

Summary: This study investigated the efficacy of endoscopic gallbladder drainage (EGBD) in patients with acute cholecystitis. The results showed no significant difference in surgery time or bleeding amount between the EGBD and percutaneous transhepatic gallbladder drainage (PTGBD) groups when surgery was performed after gallbladder drainage for acute cholecystitis. EGBD was therefore considered a useful preoperative drainage method.

Summary: This study aimed to investigate the bleeding risk associated with percutaneous transhepatic gallbladder interventions in patients with acute cholecystitis receiving antithrombotic therapy. The study included 194 consecutive patients who underwent these interventions and evaluated the risk of postoperative bleeding. The study found that the perioperative continuation of multiple antithrombotic drugs is a risk factor for postoperative bleeding after percutaneous transhepatic gallbladder interventions.

Summary: Laparoscopic cholecystectomy is the standard treatment for acute cholecystitis, but percutaneous cholecystostomy (PC) is becoming more popular due to its safety and minimally invasive nature, especially for high-risk patients. A retrospective study was conducted on patients treated with PC for acute cholecystitis, analyzing the clinical outcomes and management. The study found a moderate success rate of PC in reducing inflammation and infection, but a high mortality rate and readmission rate for biliary complications after PC.

Further improvements in techniques and training should lead to wider acceptance and dissemination of these treatment options.

Summary: This study investigated the impact of surgeon's perception of fatigue on their health during laparoscopic surgery, as well as the correlation between fatigue and operating field dimension. The results showed that surgeons experienced higher levels of fatigue when performing laparoscopic surgery involving multiple abdominal quadrants, and there was a positive correlation between duration of surgery and increase in fatigue perception.
