Summary: This study compared the diagnostic value of cytobrush, ERCP-guided biopsy, SpyGlass direct visual impression, and SpyGlass-guided biopsy (SpyBite) in differentiating between benign and malignant bile duct strictures. The results showed that SpyBite combined with SpyGlass-guided visual impression had higher sensitivity and accuracy compared to conventional endoscopic diagnostic methods such as cytobrush and standard biopsy.

Summary: Even with advanced imaging techniques, the presence of common bile duct sludge or stones can be missed during ERCP. Pre-procedural EUS or MRCP can reduce the proportion of unnecessary ERCPs. A shorter time interval between EUS or MRCP and ERCP increases the detection rate of suspected CBD stones during ERCP.

Summary: This review assesses the effectiveness and safety of the guidewire-assisted cannulation technique compared to the conventional contrast-assisted cannulation technique for the prevention of post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP). The results suggest that the guidewire-assisted cannulation technique probably reduces the risk of PEP, increases primary cannulation success, and reduces the need for precut sphincterotomy.

Summary: The study investigated the risk, predisposing factors and prognosis of pancreatitis after ERCP in elderly patients, finding that female gender, SOD, and JPD were more prevalent in elderly patients aged 65 years and older with post ERCP pancreatitis (PEP), and female gender was the most important risk factor for PEP in the elderly.

Summary: This study evaluated the factors associated with a positive cancer diagnosis from transpapillary biliary forceps biopsy (TBFB) and found that bile duct cancer, intraductal growing type, and five or more tissue samples were significant predictors of positive TBFB results. Increasing the number of tissue samples by five or more led to higher sensitivity in patients with bile duct cancer with the periductal infiltrating type.

Summary: This study aimed to examine the cumulative diagnosis rate and the predictive factors of spontaneous CBDS passage during the interval between imaging diagnosis and ERCP. The study found that CBDS sized < 6 mm on diagnostic imaging, solitary CBDS, intervals between the imaging diagnosis and ERCP, and nondilated common bile duct (< 10 mm) were significant factors associated with spontaneous CBDS passage. Therefore, preliminary endoscopic ultrasonography immediately before ERCP is recommended, especially in patients with solitary and small CBDSs on diagnostic imaging.

Summary: The study compared the efficacy and safety of LCBDE + LC with IO-ERCP + LC for the treatment of gallbladder and CBD stones, finding no significant difference in technical success, morbidity, major morbidity, and conversion rate. However, LCBDE + LC had a higher biliary leak rate, lower postoperative pancreatitis rate, and higher rate of retained stones compared to IO-ERCP + LC.

The results showed that age, concomitant heart/lung diseases, large common bile duct diameter, presence of cholelithiasis, stent/T-tube placement, and prolonged procedure increased the risk of recurrence.
