Summary: In this debate, the authors discuss the suitability of liver transplantation for patients with hepatocellular carcinoma and portal vein tumor thrombosis. The argument for liver transplantation is based on the belief that it provides greater clinical benefits in terms of survival outcomes compared to palliative systemic therapy after successful downstaging treatment. The counterargument is that the evidence for liver transplantation in this setting is limited, and expected survival in these patients may still be lower than the accepted thresholds for liver transplantation.

Summary: This study investigated the efficacy of liver-directed concurrent chemoradiotherapy (LD-CCRT) compared to sorafenib in hepatocellular carcinoma patients with portal vein tumor thrombosis (PVTT). The results showed that LD-CCRT led to significantly improved survival compared to sorafenib, and patients who underwent surgical treatment after LD-CCRT had even longer overall survival.

Summary: This study evaluated the incidence of PVT after hepatectomy for hepatocellular carcinoma and identified risk factors and predictors. The frequency of Pringle maneuver during surgery was associated with higher risk of PVT, and postoperative/preoperative ratios of TAT and D-dimer were found to be potential early predictors of PVT.

Summary: For resectable treatment-naive solitary HCC with Vp1-Vp3 PVTT, surgery may be a more suitable initial treatment compared to TACE-RT, as patients in the surgery group had higher survival rates and better preserved liver function.

Summary: ALPPS was found to result in better intermediate-term overall survival outcomes compared to TACE+PVE in patients with hepatitis B-related hepatocellular carcinoma (HCC) who were initially unresectable, although at the expense of a higher perioperative morbidity rate.

Summary: The simultaneous portal and hepatic vein embolization (PVE/HVE) appears to increase the resectability of bi-lobar liver tumors and results in faster and more pronounced hypertrophy compared with portal vein embolization (PVE) alone.

Summary: This study compared the efficacy of liver-directed combined radiotherapy (LD combined RT) with sorafenib in hepatocellular carcinoma patients presenting portal vein tumor thrombosis. The LD combined RT group showed significantly improved overall survival and a higher conversion rate to curative surgery. Despite the multimodality of the treatment, LD combined RT had comparable toxicity rates to sorafenib.

Summary: Completeness of surgery is the major prognostic factor for adrenocortical carcinoma (ACC). Specific imaging features on CT and MRI can help identify direct liver involvement (DLI) by right-sided ACC, aiding in surgical planning and reducing complication rates. Focal ACC bulge on CT and ACC contour disruption on MRI are independent and highly reproducible signs of DLI, with comparable diagnostic accuracy between the two imaging modalities.

Summary: Perihilar cholangiocarcinoma (pCCA) is a complex challenge for hepatobiliary surgeons, and poor results after ALPPS have led to exploring minimally invasive approaches and combining liver partition with PVE to improve outcomes. Robotic ALPPS shows promise in expanding surgical indications for pCCA patients, especially those with portal vein infiltration.

Summary: This study examines the impact of public insurance status on the survival outcomes of HCC patients after liver resection in China. The results show that underinsured HCC patients have worse survival outcomes after liver resection. This may be due to the limited access to care for underinsured patients, but further exploration is needed for conclusive evidence.

Summary: The study retrospectively analyzed three cases of advanced HCC with PVTT and proposed a new surgical option. The surgical procedure involved portal vein thrombectomy and liver parenchymal separation, followed by targeted drug treatment postoperatively. The results showed favorable prognosis and significantly prolonged survival time for the three patients.

Summary: This study compared the perioperative outcomes of robotic-assisted left hepatectomy or extended left hepatectomy (R-LH/R-ELH) with laparoscopic left hepatectomy or extended left hepatectomy (L-LH/L-ELH). The results showed that the robotic approach had less estimated blood loss, fewer conversions to open operations, and a shorter hospital stay compared to laparoscopic approach.

Summary: This multicentric study aimed to investigate the impact of tumor location and size on the difficulty of Laparoscopic- Left Hepatectomy (L- LH). The study found that tumors larger than 40 mm in diameter and located in PS Segment 4a were associated with the highest degree of technical difficulty. However, post-operative outcomes were not significantly different compared to smaller tumors or tumors located in other segments.
