Contributing Factors for Progression of Visual Field Loss in Normal-tension Glaucoma Patients With Medical Treatment

Purpose: To investigate the prognostic factors responsible for the progression of visual field defects (VFDs) in patients with normal-tension glaucoma (NTG) treated with topical antiglaucoma medications. Patients and Methods: A total of 92 eyes in 92 NTG patients treated with only topical antiglaucoma medications for >= 5 years were retrospectively analyzed. To identify subfield-based prognostic factors, the central 30-degree visual field (Humphrey Field Analyzer) was divided into 6 subfields: upper and lower arcuate, paracentral, and cecocentral subfields. Factors related to subfield-based progression (age, refraction, mean intraocular pressure (IOP), IOP variability, central corneal thickness, and disc hemorrhage) were evaluated using a linear mixed model. Results: Ninety-two eyes in 92 NTG patients were included in this study. The mean observation period was 7.7 +/- 2.7 (5.0 to 15.5) years, and the estimated rate of change in the mean deviation value was -0.16 +/- 0.31 dB/y (P < 0.001). A subfield-based linear mixed model analysis of the time course of changes in the mean of total deviation identified a greater extent of myopia as a significant positive prognostic factor for VFD progression in the upper paracentral area (P = 0.016). The mean IOP, central corneal thickness, disc hemorrhage, age, and IOP variation showed no significant contribution in any of the subfields. Conclusions: The extent of myopia was found to be a significant positive prognostic factor for VFD progression in the upper paracentral subfield for non-high-myopic NTG eyes with an average IOP of 14.2mm Hg under topical antiglaucoma medication.

Summary: The study showed that in otherwise healthy Japanese normal-tension glaucoma patients who received no treatment, the probability of disc haemorrhage occurrence was 31% at 5 years. Risk factors included lower myopic refraction, higher intraocular pressure during follow-up, and presence of disc haemorrhage at baseline.

Summary: This study aimed to investigate the changes in central visual field after fornix-based trabeculectomy with mitomycin C in Japanese normal-tension glaucoma patients. The results showed a significant positive change in 10-2 visual field after surgery, indicating that central vision can be maintained for NTG patients even with low preoperative IOP.

Summary: In Japanese patients with primary open-angle glaucoma (POAG) and intraocular pressure (IOP) of at most 15 mmHg undergoing prostaglandin F-2 alpha (FP) receptor agonist monotherapy, the addition of ripasudil hydrochloride hydrate led to significant IOP reduction at 1 and 3 months. Ripasudil could enhance the efficacy of FP monotherapy.

Changes in corneal endothelial cell density after initial Ex-PRESS drainage device implantation and its relating factors over 3 years

Summary: This study investigated the changes in corneal endothelial cell density (CECD) after initial Ex-PRESS surgery in Japanese patients with open-angle glaucoma (OAG) over a 36-month period. The results suggest that CECD after surgery may not pose clinical problems, but a low preoperative CECD is a significant risk factor for CECD loss. In addition, intraocular pressure (IOP) significantly decreased after surgery. Longer-term follow-up and regular CECD measurements are necessary for postoperative management, particularly in patients with low CECD.

Prostaglandin-Associated Periorbitopathy Symptom Alleviation After Switching Prostaglandin F Receptor Agonist to EP2 Receptor Agonist in Patients with Glaucoma

Summary: In this study, switching from FP agonist to EP2 agonist showed significant improvement in deepening of the upper eyelid sulcus and eyelid/peri-eyelid skin pigmentation in patients with prostaglandin-associated periorbitopathy. The treatment with EP2 agonist increased patient satisfaction and did not affect intraocular pressure or visual acuity. The improvement in pigmentation was more easily achieved in nonmyopic eyes.

Summary: This study aimed to identify factors associated with disc/retina deterioration preceding the deterioration of the visual field in patients with OAG. The results showed that disc hemorrhage was significantly associated with disc/retina deterioration preceding visual field deterioration, while age was significantly associated with visual field deterioration preceding disc/retina deterioration.

Summary: The purpose of this study was to investigate the postoperative intraocular pressure (IOP) control and identify the factors associated with failure of initial Ex-PRESS surgery in patients with open-angle glaucoma for 3 years. The study found that Ex-PRESS surgery was effective in reducing IOP, but the intervention of bleb needling after 6 months was identified as a main risk factor for achieving lower IOP.
