Summary: The functional lumen imaging probe provides objective measurements of the gastroesophageal junction during laparoscopic anti-reflux surgery. This study aimed to describe the changes in functional lumen imaging probe measurements during postoperative endoscopy and assess their long-term outcomes. The results showed that functional lumen imaging probe measurements after fundoplication were consistent over time, while patients who underwent magnetic sphincter augmentation had a tighter gastroesophageal junction.

Summary: This study compared esophageal motility disorders in patients with paraesophageal hernias (PEH) to those with sliding hiatal hernia. The results showed that PEH patients had higher rates of ineffective esophageal motility (IEM) and lower rates of absent peristalsis. High-resolution manometry (HRM) is important for preoperative assessment and procedure selection in PEH repair.

Summary: In patients with normal endoscopy, those with absent primary peristalsis and lacking a secondary peristaltic response to esophageal air distension have a more significant acid reflux burden.

Summary: The study presented a technique for circumferential esophageal hiatal rim reconstruction in 29 brachycephalic dogs with persistent regurgitation. Short-term follow-up showed a significant reduction in regurgitation frequency post surgery. It is recommended to consider this procedure for brachycephalic dogs with a history of regurgitation unresponsive to medical management.

Summary: The study followed up on Korean patients with GERD who underwent anti-reflux surgery for up to 1 year, finding that over 90% of patients had good symptom resolution at one year post-operation. The proportion of patients with hiatal hernia and comorbidities increased in the later period (2015-2018), with a significant decrease in operation time. Symptom control and complication rate remained comparable between the early and late periods.

Summary: This article provides a historical perspective on practice-changing innovations in hiatal hernia and anti-reflux surgery based on a single surgeon's experience over the past 27 years. The study reviewed 1200 operations performed by the surgeon and highlighted various advancements in surgical techniques and procedures. The findings demonstrate the impact of practice-changing innovations on anti-reflux and hiatal hernia surgery over time.

Summary: This study aimed to assess the value of transient hiatal separation during SLR in reflux patients, and found that in patients with ineffective esophageal motility, transient hiatal separation during SLR was associated with higher total acid exposure time, DeMeester scores, total acid reflux events, and longest acid reflux episodes.

Summary: This study evaluated the clinical effect of laparoscopic fundoplication on extraesophageal symptoms of gastroesophageal reflux disease (GERD). The results showed that both total and partial laparoscopic fundoplication have similar effects on improving extraesophageal symptoms of GERD, as well as similar quality of life.

Summary: This paper describes a robotic surgical technique for converting a slipped Nissen fundoplication to a Toupet fundoplication. The technique involves using robotic tools and an endoscope, and utilizing esophagogastroduodenoscopy as a bougie. This method can reduce reoperations and complications in patients undergoing fundoplication surgery.

Summary: This study systematically reviewed and meta-analyzed the efficacy of sleeve gastrectomy (SG) with concomitant hiatal hernia repair (SG + HHR) versus SG with fundoplication (SG + FP) on gastroesophageal reflux disease (GERD) remission in patients with obesity. The results showed that SG + FP had better control of GERD compared to SG + HHR, despite a higher overall complication rate. Both strategies can be considered as suitable alternatives to conventional SG in patients with obesity and concomitant hiatal hernia and/or GERD.

Summary: This retrospective study evaluated the effectiveness of a biological mesh in the 4K laparoscopic repair of hiatal hernia. The results showed that the tension-free repair with a biological mesh effectively relieved GERD symptoms and no recurrence occurred during the follow-up period, but some patients experienced dysphagia.

Summary: This study summarized the safety and efficacy of Belsey Mark IV (BMIV) transthoracic fundoplication compared to Nissen fundoplication for hiatal hernia repair. The BMIV technique showed good outcomes but had higher rates of post-operative complications and chronic pain.
