Summary: This study investigated the frequency, causative factors, and effects of acute renal cyst bleeding on the enlargement of polycystic kidneys in ADPKD patients on dialysis. The results showed that the total volume of acute renal cyst bleeding was significantly associated with total kidney volume and was greater in younger patients. These findings suggest that renal cyst bleeding and renal artery blood flow may synergistically accelerate the enlargement of polycystic kidneys in ADPKD patients on dialysis.

Summary: This study aims to evaluate the role of hepatic venous pressure gradient (HVPG) in predicting further decompensation in cirrhosis patients with acute variceal bleeding (AVB). The study found that 27.6% of patients developed further decompensation within 42 days after the bleeding episode, with gastrointestinal bleeding and new-onset/worsening ascites being the most common events. HVPG was found to be an independent predictor of further decompensation in patients with esophageal variceal bleeding.

Summary: This study compared the clinical outcomes of marginal ulcer bleeding (MUB) with peptic ulcer bleeding (PUB). The results showed that MUB had a higher rebleeding rate and a higher mortality rate compared to PUB. Prophylactic use of proton pump inhibitors (PPIs) may reduce the risk of rebleeding in MUB.

Summary: This study explored how patients and caregivers select hospitals for complex cancer care and identified six broad domains that characterized hospital selection considerations, including hospital factors, team characteristics, travel distance, referral or recommendation, continuity of care, and insurance considerations. The study found that participants at different types of medical centers had similar considerations but with some differences, such as access to clinical trials for participants at metropolitan centers and insurance considerations for participants at urban/suburban centers.

Summary: The current AJCC T stage classification for gastric neuroendocrine tumors has been shown to have poor prognostic discriminability. A modified T-stage classification was proposed in this study, demonstrating significantly improved stratification for patients. Summary: Intraductal papillary mucinous neoplasms (IPMNs) are commonly diagnosed and range from benign to malignant lesions. Current prediction of high-grade dysplasia and invasive cancer relies on clinical and radiographic features to guide treatment decisions. However, the natural history of IPMNs is not completely understood and guidelines from different societies provide conflicting recommendations, making it challenging to balance the risk of missing cancer with surveillance and surgical resection morbidity. This review aims to reconcile guideline differences, provide a clinical framework for IPMN management, and highlight advances to minimize missed cancers and unnecessary operations.
