Summary: This study focused on minimizing the number of anastomoses, restoring intestinal continuity, and avoiding stoma creation in 295 patients with stage III/IV ovarian cancer undergoing low anterior rectal resection (LAR). Various techniques were used for tension-free anastomosis when the remaining colon could not reach the rectal stump after left hemicolectomy with LAR, showing that colorectal anastomosis is feasible for patients with extended colonic resection, leading to low rates of anastomotic leakage and stoma creation.

Summary: This study demonstrated the significance of MRI-based radiomics model in differentiating local recurrence of rectal cancer from nonrecurrence lesions at the site of anastomosis. The combination of multiple sequences significantly improved the model's performance, showing higher sensitivity and specificity in the validation set. The decision curve analysis confirmed the clinical usefulness of the radiomics model.

Summary: This study aimed to establish the safe values for quantitative perfusion parameters in laparoscopic colorectal surgery. By comparing tissue oxygenation levels from HSI images, the researchers found that T-1/2MAX ≤ 10 s, T-MAX ≤ 30 s, slope ≥ 5, and NIR perfusion index ≥ 50 were associated with colon tissue oxygenation levels higher than 60%. The diagnostic values of T-1/2MAX, T-MAX, and perfusion TR showed high sensitivity values (≥ 97%), indicating their ability to correctly identify cases with acceptable tissue oxygenation.

Endoscopic techniques, such as balloon dilatation and stenting, show varying success rates in avoiding the need for surgical reintervention, with challenges including the need for repeated treatments and stent migration. Further research is needed to explore novel therapies for improving quality of life in these patients.

Summary: Development of an anastomotic leak (AL) following colorectal cancer surgery is a life-threatening complication that can lead to severe consequences such as abdominal contamination and peritonitis. Current research is focusing on identifying diagnostic or predictive biomarkers for AL to enhance post-operative care and improve outcomes for patients. Although no AL biomarker has been validated in large-scale clinical trials yet, personalized medicine through biomarker analysis could hold promise for colorectal cancer patients undergoing intestinal resection and anastomosis in the future.
