The role of peroral video cholangioscopy in patients with IgG4-related sclerosing cholangitis

The cholangioscopic features of IgG4-related sclerosing cholangitis (IgG4-SC) remain undefined. The aim of this study was to clarify these endoscopic features using peroral video cholangioscopy (PVCS) in IgG4-SC patients. PVCS was performed in 33 patients: IgG4-SC (n = 13); primary sclerosing cholangitis (PSC; n = 5); and cholangiocarcinoma (n = 15), which included hilar cholangiocarcinoma (HCCA; n = 5) and distal cholangiocarcinoma (DCCA; n = 10). The most frequent findings on PVCS in the IgG4-SC patients were dilated (62 %) and tortuous (69 %) vessels, and absence of partially enlarged vessels. The incidence of dilated and tortuous vessels was significantly higher in IgG4-SC patients than in PSC patients (p = 0.015). Scarring and pseudodiverticula were found significantly more often in PSC patients than in IgG4-SC patients (p = 0.001 and p = 0.0007, respectively). The incidence of partially enlarged vessels was significantly higher in DCCA patients than in IgG4-SC patients (p = 0.004). In contrast, the incidence of dilated vessels was significantly higher in IgG4-SC patients than in HCCA patients (p = 0.015). PVCS performed after corticosteroid therapy showed resolution of bile duct stenosis and dilated, tortuous, or partially enlarged vessels, as well as resolution of friability in all patients with IgG4-SC. Cholangioscopy was useful in differentiating IgG4-SC from PSC. In addition, monitoring the patterns of proliferative vessels on PVCS may be useful to differentiate IgG4-SC from cholangiocarcinoma.

Summary: In this study, the effect of lenvatinib on tumor blood vessel diameter in patients with hepatocellular carcinoma was investigated. It was found that lenvatinib can reduce the diameter of tumor blood vessels, which may be considered as a process of normalization. The decrease in tumor blood vessels in imaging analysis may be associated with improved prognosis, but further studies are still required.

Summary: This study aimed to identify factors associated with the induction of third-line chemotherapy for advanced gastric cancer, focusing on patient eligibility for clinical trial. The results showed that factors such as ineligibility for clinical trials, number of metastatic sites, low albumin concentration, and a lack of response to first-line treatment were associated with the initiation of third-line chemotherapy. Additionally, the study suggested that patients who responded to first-line treatment had more favorable prognoses when treated with salvage chemotherapy, even if they did not meet the eligibility criteria for clinical trials.

Summary: This study investigated the regulatory mechanisms and expression of TRPV4 in colon epithelial cells co-cultured with various intestinal bacteria. The results showed that co-culture with certain bacteria temporarily decreased TRPV4 expression, while co-culture with other bacteria increased TRPV4 expression. Additionally, TRPV4 expression was higher in constipated patients and correlated with increased frequency of E. faecalis.

Updates in narrow-band imaging for colorectal polyps: Narrow-band imaging generations, detection, diagnosis, and artificial intelligence

Summary: Narrow-band imaging (NBI) is an optical digital enhancement method that allows the observation of vascular and surface structures of colorectal lesions. Despite some issues in earlier models, NBI has evolved and the newest endoscopic platform offers significantly brighter and detailed images. Increased visualization and universal compatibility across all scopes are expected to improve the detection and diagnosis of polyps and promote global standardization. Knowledge of NBI and magnifying colonoscopy will become essential for future endoscopic practice.

A ruptured sarcomatoid hepatocellular carcinoma treated with combined immunotherapy

Summary: Sarcomatoid hepatocellular carcinoma (sHCC) is a rare and highly aggressive form of liver cancer that has no established pharmacological treatment. This study reports a case of sHCC that emerged two years after radiofrequency ablation (RFA), but was managed successfully with interventional therapies and immunotherapy. The findings suggest that combined immunotherapy may be effective in treating sHCC, but further research is needed to determine its efficacy in a larger patient population.

Summary: This multicenter study evaluated the long-term changes in thrombocytopenia and leukocytopenia in HCV-cirrhosis patients treated with DAAs. The results showed that DAA treatment improved thrombocytopenia and leukocytopenia and also led to a reduction in liver fibrosis markers and spleen size over a 5-year period.

Summary: A study of 357 patients receiving EUS-guided treatment for pancreatic fluid collections (PFCs) found that adverse events were more likely to occur in patients with walled-off necrosis (WON) and risk factors included WON extension to the pelvis and endoscopic necrosectomy. Higher Charlson Comorbidity Index, extension to the pelvis, nonuse of a lumen-apposing metal stent, and percutaneous drainage were risk factors for clinical failure in WON treatment. Patients with pseudocysts extending to the paracolic gutter and those who needed more than two endoscopic/percutaneous procedures had a higher risk of clinical failure.
