Summary: Peroral Cholangioscopy (POCS) is effective in achieving complete stone removal for intrahepatic stones, but it is associated with a longer procedure time compared to fluoroscopic guidance. However, the incidence of cholangitis was not significantly different between patients with and without POCS. This study highlights the importance of specialized equipment and experienced staff for endoscopic treatment of intrahepatic stones.

Summary: This study aimed to examine the cumulative diagnosis rate and the predictive factors of spontaneous CBDS passage during the interval between imaging diagnosis and ERCP. The study found that CBDS sized < 6 mm on diagnostic imaging, solitary CBDS, intervals between the imaging diagnosis and ERCP, and nondilated common bile duct (< 10 mm) were significant factors associated with spontaneous CBDS passage. Therefore, preliminary endoscopic ultrasonography immediately before ERCP is recommended, especially in patients with solitary and small CBDSs on diagnostic imaging.

Summary: This study aimed to investigate the efficacy and safety of EHL using peroral cholangioscopy (POCS) under endoscopic retrograde cholangiopancreatography (ERCP) guidance in older adults aged >= 80 years. The results showed that complete removal of common bile duct stones was achieved in 93.8% and 100% of elderly and non-elderly groups, respectively. The occurrence of adverse events during the EHL session did not significantly differ between the two groups.

Summary: We developed a feasible endoscopic classification system based on peroral cholangioscopy (POC) findings for intraductal neoplasm of the bile duct (IN-B). POC with narrow-band imaging (NBI) was found to be useful for detecting IN-Bs. Our novel classification system, which considers both surface structure and microvascular pattern, allows differentiation of IN-B from intraductal superficial lesions of the bile duct (ISL-Bs). The article highlights the importance of considering biliary tree disruption in patients with right upper quadrant discomfort following an iatrogenic or traumatic event, and suggests that a combination of radiological imaging and minimal invasive techniques can be successful in managing biloma.

Summary: This study aimed to explore the alteration of bile acid profiles in patients with choledocholithiasis (CDC) and construct a prediction model for evaluating the risk of common bile duct stone (CBDS) recurrence after endoscopic treatment. The study found that the serum BA profiles were different in patients with CDC compared to controls, and a diagnostic model was successfully developed to distinguish patients with CDC from controls. The developed nomogram model for predicting the risk of CBDS recurrence had high accuracy and clinical usability.

The results showed no significant difference in treatment outcomes between LCBDE and ERCP. However, LCBDE had a higher rate of bile leakage and a lower rate of postoperative pancreatitis, cholangitis, and hemorrhage.

Summary: This study aimed to identify factors associated with the induction of third-line chemotherapy for advanced gastric cancer, focusing on patient eligibility for clinical trial. The results showed that factors such as ineligibility for clinical trials, number of metastatic sites, low albumin concentration, and a lack of response to first-line treatment were associated with the initiation of third-line chemotherapy. Additionally, the study suggested that patients who responded to first-line treatment had more favorable prognoses when treated with salvage chemotherapy, even if they did not meet the eligibility criteria for clinical trials.

Summary: This study investigated the regulatory mechanisms and expression of TRPV4 in colon epithelial cells co-cultured with various intestinal bacteria. The results showed that co-culture with certain bacteria temporarily decreased TRPV4 expression, while co-culture with other bacteria increased TRPV4 expression. Additionally, TRPV4 expression was higher in constipated patients and correlated with increased frequency of E. faecalis.

Summary: Narrow-band imaging (NBI) is an optical digital enhancement method that allows the observation of vascular and surface structures of colorectal lesions. Despite some issues in earlier models, NBI has evolved and the newest endoscopic platform offers significantly brighter and detailed images. Increased visualization and universal compatibility across all scopes are expected to improve the detection and diagnosis of polyps and promote global standardization. Knowledge of NBI and magnifying colonoscopy will become essential for future endoscopic practice.

Summary: Sarcomatoid hepatocellular carcinoma (sHCC) is a rare and highly aggressive form of liver cancer that has no established pharmacological treatment. This study reports a case of sHCC that emerged two years after radiofrequency ablation (RFA), but was managed successfully with interventional therapies and immunotherapy. The findings suggest that combined immunotherapy may be effective in treating sHCC, but further research is needed to determine its efficacy in a larger patient population.

Summary: A study of 357 patients receiving EUS-guided treatment for pancreatic fluid collections (PFCs) found that adverse events were more likely to occur in patients with walled-off necrosis (WON) and risk factors included WON extension to the pelvis and endoscopic necrosectomy. Higher Charlson Comorbidity Index, extension to the pelvis, nonuse of a lumen-apposing metal stent, and percutaneous drainage were risk factors for clinical failure in WON treatment. Patients with pseudocysts extending to the paracolic gutter and those who needed more than two endoscopic/percutaneous procedures had a higher risk of clinical failure.
