Summary: BOM is a common adverse event after myotomy for achalasia, associated with post-treatment symptom severity, achalasia type, and treatment modality. Pretreatment type III achalasia, LHM as opposed to POEM, and an increased post-treatment integrated relaxation pressure are risk factors for BOM development.

Summary: Screening and surveillance of POEM are crucial in detecting esophageal carcinoma. ESD is feasible for achalasia cases, but can be challenging on the POEM-line due to fibrosis. Surveillance endoscopy is recommended for specific high-risk cases and those with a history of esophageal carcinomas.

Summary: Comparing clinical and histopathologic features between untreated achalasia and refractory GERD patients, it was found that achalasia patients had higher Eckardt scores, more hypertrophy of the muscularis mucosae and petechiae formation, and esophageal food retention. A combination of Eckardt score, endoscopic features, and histopathology from esophageal mucosal biopsy could provide earlier identification of achalasia.

Summary: Primary malignant melanoma of the esophagus is a rare but highly aggressive tumor, diagnosis requires immunohistochemical staining and comprehensive examination. Early detection and radical resection of the tumor are crucial for treatment and prognosis.

Summary: Endoscopic suspicion of residual tumor was the only endoscopic finding associated with residual disease. Based on its positive predictive value, this endoscopic finding may contribute to the diagnostic strategy used in active surveillance.

Summary: To ensure a pathologically negative proximal margin (PM) in gastric cancer with esophageal invasion or esophagogastric junction cancer, the esophagus should be transected beyond the gross boundary of invasion. However, there are no established recommendations for the length of esophageal resection in these cases.

Summary: This study examined the relationship between distensibility index (DI) and integrated relaxation pressure (IRP), and their correlations with dysphagia symptoms in patients with achalasia and esophagogastric junction outflow obstruction (EGJOO). The study found that DI is not correlated with HRM EGJ measurements and has the strongest effect on dysphagia severity.

Summary: Peroral endoscopic myotomy (POEM) is an effective and safe treatment for esophageal achalasia and related diseases in elderly patients aged 75 years and over, with a high procedural success and clinical success rate.

Summary: The accuracy of preoperative endoscopic evaluations of EIL in EGJ adenocarcinoma is around 53.3%, with underestimation being common. Factors like undifferentiated histologic type and pN1-3 play a role in this underestimation, while EIL measurements of 2-3 cm and >3 cm are independent predictors of metastasis or recurrence in MLNs.

Summary: Smooth muscle fibrosis is common in the lower part of the esophagus in patients with achalasia, and it is positively correlated with the severity of symptoms two years after per-oral endoscopic myotomy (POEM). The fibrosis may be related to eosinophil infiltration and TGF-beta 1.

Summary: This study compared the prognosis and lymph node metastasis risk in patients with early-stage EGJ adenocarcinoma after endoscopic treatment or radical surgery. Logistic regression analysis identified independent predictors of LNM. Cox regression analysis showed no significant difference in CSS between ET and radical surgery for early-stage EGJ adenocarcinoma patients.

Summary: More than 20% of achalasia patients show signs of partial recovery of esophageal peristalsis after POEM treatment. This recovery is mainly seen in patients with type II achalasia, but the clinical impact appears to be minimal.

Summary: This study aimed to compare the efficacy and complications of the two main treatments for achalasia: endoscopic dilatation and surgical cardiomyotomy (Heller's myotomy). After adjusting for potential confounding factors, Heller's myotomy was significantly more successful than endoscopy dilatation, with no significant difference in the occurrence of serious complications.
