Summary: This study found that not all patients with nighttime reflux symptoms have pathological acid exposure on every night. Nighttime acid exposure variability is greater than upright acid exposure, and the diagnostic accuracy of pH monitoring within 24 hours is low, improving with longer testing duration.

Summary: This study investigated the prevalence of pediatric non-erosive esophageal phenotypes (NEEPs) and their treatment response. The results showed that functional heartburn may be the most common pediatric NEEP. Long-term follow-up revealed a higher rate of symptom resolution with proton pump inhibitor (PPI) therapy in non-erosive reflux disease (NERD) patients, while other groups did not benefit from extended acid-suppressive treatment.

Summary: Tegoprazan showed superior therapeutic efficacy compared with the placebo in Korean patients with NERD, especially in complete resolution of major symptoms, with a favorable safety profile and tolerability.

Summary: This study compared dietary and lifestyle triggers between non-erosive reflux disease (NERD) and reflux esophagitis (RE) in Chinese patients and provided evidence for practical dietary modifications for GERD.

Summary: This study showed that in patients with typical reflux symptoms, inadequate PPI response, and absence of severe esophagitis, acid exposure on reflux monitoring could predict the ability to discontinue PPIs without symptom escalation. Symptom burden reduction was greater among those who discontinued PPI compared to those who resumed. It is suggested that upfront reflux monitoring off acid suppression can help limit unnecessary PPI use and guide personalized management.

Summary: Abnormal metrics identified in pH impedance studies performed while on PPI therapy are associated with nonresponse in patients with proven GERD, providing guidance for treatment escalation, including surgical intervention, especially in regurgitation-predominant GERD cases.

Summary: S-isomer (S) pantoprazole at a low dose (10 mg) for 4 weeks showed higher efficacy than placebo in providing symptom relief for patients with non-erosive reflux disease (NERD), particularly in acid regurgitation. Older age, female gender, higher body mass index, and severe baseline symptoms were associated with poor responsiveness to proton pump inhibitors.

Summary: Fexuprazan 40 mg is non-inferior to esomeprazole 40 mg in healing erosive esophagitis at week 8. The study suggests that Fexuprazan may be a promising alternative treatment option to PPIs for patients with erosive esophagitis.

Summary: This study confirmed the effectiveness of a simple method in diagnosing reflux disease by measuring mean nocturnal baseline impedance in patients with laryngopharyngeal reflux and gastroesophageal reflux disease. The results showed a high level of agreement between the two methods in the distal esophagus, and moderate agreement in the proximal esophagus and pharynx.

Summary: By comparing psychological comorbidities and gastroesophageal reflux measurements in patients with reflux hypersensitivity, functional heartburn, and non-erosive reflux disease, it was found that these patients had heightened visceral sensitivity and psychological factors such as anxiety and depression played a significant role in these conditions.

Summary: This study demonstrates that increased pain perception in patients with nonerosive reflux disease may result from the expression of acid-sensitive channels on superficial mucosal afferents and esophageal epithelial cells. This raises the potential for topical therapy as a treatment option.

Summary: Gastro-esophageal reflux disease (GERD) affects approximately 25% of the general population. Due to the complexity of the disease, there is no singular diagnostic test available. Most GERD patients do not have visible lesions, known as non-erosive reflux disease (NERD). Currently, 24-hour impedance-pH monitoring is the best diagnostic tool to detect abnormal reflux and correlate symptoms. Novel impedance metrics have shown potential to improve the accuracy of diagnosis.
