Morphologic Micro-Computed Tomography Analysis of Mandibular Premolars with Three Root Canals

Introduction: This study aimed to describe the anatomy of mandibular premolars with type IX canal configuration by using micro computed tomography. Methods: Mandibular premolars with radicular grooves (n = 105) were scanned, and 16 teeth with type IX configuration were selected. Number and location of canals, distances between anatomic landmarks, occurrence of apical delta, root canal fusion, and furcation canals, as well as 2-dimensional (area, perimeter, roundness, major and minor diameters) and 3-dimensional (volume, surface area, and structure model index) analysis were performed. Data were statistically compared by using analysis of variance and Kruskal-Wallis tests (alpha = 0.05). Results: Overall, specimens had 1 root with a main canal that divided into mesiobuccal, distobuccal, and lingual canals at the furcation level. Mean length of the teeth was 22.9 +/- 2.06 mm, and the configuration of the pulp chamber was mostly triangle-shaped. Mean distances from the furcation to the apex and cementoenamel junction were 9.14 +/- 2.07 and 5.59 +/- 2.19 mm, respectively. Apical delta, root canal fusion, and furcation canals were present in 4, 5, and 10 specimens, respectively. No statistical differences were found in the 2-dimensional and 3-dimensional analyses between root canals (P > .05). Conclusions: Type IX configuration of the root canal system was found in 16 of 105 mandibular premolars with radicular grooves. Most of the specimens had a triangle-shaped pulp chamber. Within this anatomic configuration, complexities of the root canal systems such as the presence of furcation canals, fusion of canals, oval-shaped canals in the apical third, small orifices at the pulp chamber level, and apical delta were also observed.

Summary: Extending the use of heterologous fibrin biopolymer, this pre-clinical study examined the formation of a less dense scaffold to facilitate bone cell infiltration by adjusting the proportionality of its components. Combined with laser photobiomodulation therapy, it aimed to accelerate the repair process and reduce recovery time. The study found that the laser-treated groups showed higher bone volume and bone area, indicating the potential effects of laser radiation in promoting bone regeneration.

Success rate of permanent teeth pulpotomy using bioactive materials: A systematic review and meta-analysis of randomized clinical trials

Summary: This systematic review and meta-analysis show that pulpotomy is a highly effective treatment for managing permanent teeth. The results indicate that the success rate of pulpotomy using ProRoot MTA is significantly higher than when using calcium hydroxide. However, the certainty of evidence supporting these findings is low, and there is a need for well-designed RCTs to assess the long-term outcomes of pulpotomy using newer bioceramic materials.

Multimethod analysis of three rotary instruments produced by electric discharge machining technology

Summary: This study aimed to compare three rotary instruments produced by the EDM process with the heat-treated ProTaper Gold system regarding design, metallurgy, mechanical properties and shaping ability. The results showed that HyFlex EDM, Neoniti, and EDMax rotary instruments had similar designs, metallurgy, and mechanical performances when compared with ProTaper Gold. However, there were no differences in their shaping ability.

Effect of the progressive taper enlargement of buccal root canals of three-rooted maxillary molars: A stepwise micro-CT study

Summary: This study evaluated the preparation status, dentine removal volume, and dentine thickness of buccal canals in 22 maxillary molars with and without the MB2 using instruments with the same tip size but different tapers. Micro-CT scans were performed after each instrument. The results showed that there was an increase in dentine removal volume and a decrease in unprepared areas and dentine thickness after each treatment step, with higher reduction in dentine thickness in the MB2 canal compared to the mesiobuccal and distobuccal canals. Knowledge of pre-operative dimensions and dentine thickness is crucial to avoid excessive dentine removal.

Evaluation of cyclic and torsional fatigue resistance of several heat-treated reciprocating nickel-titanium instruments

Summary: This study investigated the cyclic fatigue and torsional resistance of different rotary files at body temperature. Reciproc Blue had the highest time to fracture, followed by Wave One Gold and Unicone Plus. There was no significant difference in the number of cycles to fracture between Reciproc Blue and Wave One Gold. Unicone showed the highest torque values and the lowest angular deflection. SEM analysis revealed typical failures features in both cyclic and torsional fatigue tests.

Quasi-3D dynamic photoelastic analysis of stress distribution during preparation of simulated canals with 13 mechanical preparation systems

Summary: The aim of this study is to compare the stress produced on the internal walls of simulated canals by different rotary and reciprocating systems. The stress generated on the canal walls were evaluated using a semi-quantitative scale by two independent observers. The results showed that instruments with larger taper, lower speed, reciprocating motion, and made of heat-treated NiTi alloy exhibited higher patterns of stress distribution on the canal walls.

To flat or not to flat? Exploring the impact of flat-side design on rotary instruments using a comprehensive multimethod investigation

Summary: This study aims to assess the influence of a flat-side design on the geometry, metallurgy, mechanical performance, and shaping ability of a novel nickel-titanium rotary instrument. The results showed that the flat-side instrument demonstrated enhanced cutting ability compared to its nonflat counterpart, but inferior performance in terms of time, rotation, and maximum torque to fracture, along with distinct phase transformation temperatures. No significant differences were observed in other aspects between flat and nonflat instruments.

Summary: This study evaluated the cyclic and torsional fatigue resistance of a new nickel-titanium instrument. It found that the Flat File 25.04 had lower cyclic fatigue and reciprocating motion improved the cyclic fatigue resistance. The Flat File 25.04 also showed good resistance to torsional fatigue.
