Effect of levothyroxine replacement on exercise performance in subclinical hypothyroidism

Although muscle metabolism and exercise capacity seem to be affected in patients with subclinical hypothyroidism, there is little evidence indicating improvement of the exercise tolerance due to levothyroxine (L-T-4) replacement. The aim of the present study was to verify possible cardiopulmonary changes during exercise in patients with subclinical hypothyroidism on L-T-4 replacement with a normal serum TSH for six months. Twenty-three patients with subclinical hypothyroidism were randomized into treated (no.=11) and untreated (no.=1 2) patients. A cardiopulmonary test was performed with a treadmill, using the modified Balke protocol. Heart rate, oxygen uptake, minute ventilation and other cardiopulmonary parameters were assessed at the 5th minute of exercise. FT4 levels increased while TSH normalized after hormone replacement. Oxygen uptake decreased significantly after hormone replacement (24.1 +/- 6.3 vs 17.1 +/- 4.2 ml.kg.min(-1); p=0.03). Minute ventilation also showed an enhanced performance in treated patients (28.0 +/- 8.1 vs 23.5 +/- 5.6 l.min-1; p=0.03), as did the heart rate (128 +/- 17 vs 121 +/- 17 bpm; p=0.03). There were no changes in the untreated group. The results demonstrate that submaximal cardiopulmonary exercise performance improved after six months of TSH normalization and this improvement can help enhance the ability to carry out daily life activities in patients with subclinical hypothyroidism. Summary: The study evaluated the cancer risk of indeterminate thyroid nodules by considering both cytological subcategories and the American Thyroid Association (ATA)-based US risk classification. The highest RoM was related to nuclear atypia (NA) and both architectural and nuclear atypia (ANA) subcategories, while architectural atypia (AA) and oncocytic pattern (OP) had lower RoM. NA and ANA were independently associated with cancer, as were high and intermediate-risk US categories according to ATA. The presence of NA or ANA combined with intermediate US patterns had high RoM values, and combining cytological features with high-risk US categories resulted in RoM exceeding 70%.

Summary: TSI and TBII assays showed similar performance in differentiating GD from other causes of thyrotoxicosis. In combination, both tests showed little benefit compared with the TBII assay alone. Thyrotropin receptor antibody assay results should be carefully interpreted in patients with mild GD or longstanding disease.

Summary: This study investigated the relationship between thyroid function and the risk of developing diabetes. The results suggest that fT4 and fT3 levels, as well as their conversion ratio, may be associated with incident diabetes, especially in the presence of overweight or obesity. However, further studies are needed to confirm these findings.

Does a three-degree hypoechogenicity grading improve ultrasound thyroid nodule risk stratification and affect the TI-RADS 4 category? A retrospective observational study

Summary: The study aimed to determine if classifying hypoechogenicity into three degrees could improve the distinction between benign and malignant nodules and influence the Thyroid Imaging Reporting and Data System (TI-RADS) Category 4. A total of 2,574 nodules were retrospectively assessed, and a subanalysis was performed on solid nodules to evaluate mainly TI-RADS 4 nodules. The results showed that mild hypoechogenicity had a lower association with malignancy compared to moderate and marked hypoechogenicity, and there was no significant association between mildly hypoechoic solid nodules and cancer.

Free thyroxine but not TSH levels are associated with decline in functional status in a cohort of geriatric outpatients

Summary: The study showed that in older patients, for each 0.1 ng/dL increase in FT4 levels, the risks of functional decline increased by 14.1% and 7.7% for baseline and mean FT4 follow-up levels, respectively. Patients with baseline FT4 levels in the fourth and fifth quintiles had a nearly 9 times higher risk of functional decline compared to those in the first quintile. No association was found between TSH and the outcome.
