The biological characterization of neuroendocrine tumors: The role of neuroendocrine markers

Neuroendocrine tumors (NET) may originate in different organs, from cells embriologically different but expressing common phenotypic characteristics, such as: the immuno-reactivity for markers of neuroendocrine differentiation (defined as pan-neuroendocrine), the capacity to secrete specific or aspecific peptide and hormones and the expression of some receptors, that are at the basis of the current diagnostic and therapeutical approach, peculiar to these tumors. NET have been conventionally distinguished in functioning, when associated with a recognized clinical endocrine syndrome, and non-functioning. However, this terminology may be misleading, since the great majority of NET may secrete neuroendocrine peptides, which can be employed as clinical markers for both diagnosis and follow-up. On the other hand, tissue immuno-reactivity for specific hormones does not always reflect secretory activity of the tumor cells. Finally, receptors and genetic markers are acquiring a relevant role in the characterization of NET, both improving knowledge of biology and physiopathology of NET, as well as in developing specific strategies to establish an early diagnosis and targeted therapies, to adopt prophylactic strategies in familial forms, and to identify more efficacious targets for therapy in the future.

Summary: This review reevaluates the genetic basis and clinical features of gastroenteropancreatic and thoracic neuroendocrine tumors (NETs) in the context of inherited syndromes, with a special focus on molecularly targeted therapies. Significant advances have been made in understanding the genetic and epigenetic mechanisms of NET development, leading to the identification of targetable alterations and the development of novel targeted therapies.

Surgery is the main treatment for local disease, while non-resectable cases have multiple therapeutic options. This review provides a summary of the clinical management and therapeutic approaches for functional pNENs.

Summary: The study aimed to evaluate glucose control and insulin regimen modifications during pregnancy in women with T1D, comparing them to non-diabetic, normal-weight pregnant women. The findings showed that women with T1D need to modify their insulin regimen and dietary habits to control weight gain and promote optimal birth outcomes. However, due to the increase in insulin regimen, women with T1D gave birth to babies with higher birth weights than non-diabetic pregnant women.

Carcinoid Syndrome: Preclinical Models and Future Therapeutic Strategies

Summary: Carcinoid syndrome is a debilitating paraneoplastic disease occurring in 10-40% of patients with neuroendocrine tumors. Its main symptoms include flushing, diarrhea, hypotension, tachycardia, bronchoconstriction, venous telangiectasia, dyspnea, and fibrotic complications. Although there are drugs available, lack of therapeutic response and resistance remain challenges. Preclinical models serve as crucial tools for studying tumor progression and developing new therapies. This paper provides an overview of in vitro and in vivo models in NETs with carcinoid syndrome and discusses future developments and therapeutic approaches.

Summary: In this study, alternative criteria for evaluating the efficacy of systemic treatments in patients with neuroendocrine tumors (NETs) and liver metastases (LMs) were explored. It was found that a reduction of ≥10% in the size of three LMs predicted prolonged treatment duration, which is more clinically relevant than the current threshold of 30% utilized by RECIST 1.1.

Summary: The aim of this study was to evaluate the predictive role of specific clinical factors for the diagnosis of MEN1 and MEN4 in patients with an initial diagnosis of gastrointestinal, bronchial, or thymic neuroendocrine tumor (NET). The preliminary data showed that a diagnosis of NET in patients with a negative family history may suggest MEN1 in the presence of >= three positive phenotypic criteria.

This study found that NENs patients have an immunosuppressed environment, characterized by a decrease in cytotoxic natural killer cell count, an increase in anti-inflammatory non-classical monocytes count, and a decrease in T-helper lymphocyte count.
