Background: Merkel cell carcinoma is a rare, aggressive, malignant cutaneous tumor of the elderly or immunosuppressed individuals that usually appears on sun-exposed areas of the body. Its pathogenesis is still debated, and, currently, no standardized treatment exists. Objective: To provide a current updated review of the most relevant data concerning the pathogenesis and management of Merkel cell carcinoma. Methods: Using relevant MeSH terms, we performed a review of the literature on these subjects from 1980 to June 2009. Results and Conclusion: The current management of Merkel cell carcinoma is based on surgical excision as the majority of patients present with localized disease, whereas up to 30% have regional lymph node metastases. In these cases, the best outcome is achieved with multidisciplinary management that includes radiotherapy. Chemotherapy is part of the treatment in advanced cases and is mandatory for distant metastatis. Given that a recent work showed the presence of a previously unknown polyomavirus, which the authors called Merkel cell polyomavirus, the therapeutical approach to Merkel cell carcinoma could be reconsidered in the future.

Summary: Retinoids and antibiotics are commonly used as topical treatments for mild to moderate acne, with irritant contact dermatitis being a common side effect. Short contact therapy (SCT) may improve local tolerability, and a gel containing tretinoin, clindamycin, and glycolic acid has shown to be effective in acne treatment. The SCT approach could potentially improve skin tolerability and adherence to treatment.

Summary: A case of lentigo maligna melanoma of the scalp in an elderly patient was treated with surgery and subsequent application of 5% and 3.75% Imiquimod, leading to complete regression of the lesion. 3.75% Imiquimod, already used for actinic keratosis, could be a useful and less side effect treatment option.

Acral perineurioma: A case report of a rare neoplasm successfully treated with autologous skin graft reconstructive surgery

Summary: Soft tissues perineurioma is a rare benign nerve sheath tumor that usually affects the acral sites of young people. Complete surgical excision and reconstructive surgery may be challenging, but can lead to full functional recovery. We report a case of successful radical surgical excision with autologous full-thickness skin graft resulting in complete functional recovery.

Summary: The background provides detailed information about bullous pemphigoid (BP) and its impact on patients. The objective of the study is to update the guidelines for managing this disease. The results suggest that treatment should be tailored based on the severity of BP and patients' comorbidities. High-potency topical corticosteroids are recommended as the main treatment, while the use of doxycycline and dapsone is controversial. B-cell-depleting therapy and intravenous immunoglobulins can be considered in treatment-resistant cases. The final version of the guideline received consent from several patient organizations.
