Abstract

In recent years, the use of antibodies against tumor necrosis factor alpha (TNF alpha) has expanded in rheumatology, gastroenterology, and dermatology. In addition to the more common side effects such as infections and hypersensitivity reactions, elevations of liver enzymes have been reported during anti-TNF alpha therapy, although severe liver failure has been extremely uncommon. This report describes a patient with severe liver failure after induction therapy with the TNF alpha antibody infliximab (Remicade (R)). A 46-year old female patient received two infusions of infliximab at a dose of 400 mg on weeks 0 and 8 for steroid-dependent ulcerative colitis. Eight weeks after the second infusion, she suffered acute liver failure with necrosis, requiring liver transplantation. (C) 2012 European Crohn's and Colitis Organisation. Published by Elsevier B.V. Tebuconazole induces liver injury coupled with ROS-mediated hepatic metabolism disorder

Summary: The study found that low doses of tebuconazole mainly accumulated in the liver of mice, causing histopathological damage. Tebuconazole significantly dysregulated metabolizing enzymes, efflux transporters, and fatty acid metabolism-related genes in the liver, leading to liver hypertrophy and steatosis. Furthermore, excessive induction of ROS and oxidative stress partly accounted for the metabolic abnormalities mediated by tebuconazole.

Proteomic analysis of hepatic effects of phenobarbital in mice with humanized liver

Summary: This study used a mouse model with humanized livers to investigate the effects of the model CAR activator phenobarbital (PB) on human and mouse hepatocytes. The proteomic analysis showed similar responses in nuclear receptor activation and xenobiotic metabolism between human and mouse hepatocytes. However, activation of the MYC proliferation regulator was only predicted in mouse hepatocytes, not in human hepatocytes. This study provides comprehensive proteomic evidence that PB induces adaptive metabolic responses but not hepatocellular proliferation in human hepatocytes in vivo.

Hepatic presentations of mitochondrial DNA depletion syndrome in children: A single tertiary liver centre experience

Summary: Mitochondrial DNA depletion syndromes (MDDS) are a heterogeneous group characterized by variable hepato-cerebral phenotypes. A retrospective study analyzed 24 MDDS patients from January 2002 to September 2019, identifying 7 POLG, 7 DGUOK, and 10 MPV17 cases. Presentation age ranged from 0.06 to 189 months, with 16 patients experiencing acute liver failure (ALF) and 8 with chronic cholestasis and/or elevated transaminases. Liver involvement onset varied among different gene mutations. Neurological involvement was observed in 18 patients. Liver histology revealed necrosis, steatosis, cholestasis, and fibrosis in varying degrees. Genetic testing facilitated liver transplant in MPV17 patients. However, decision-making for infantile ALF cases remains challenging, highlighting the importance of rapid genetic testing.

Extracorporeal liver support and liver transplantation for acute-on-chronic liver failure

Summary: Acute-on-chronic liver failure (ACLF) is a life-threatening condition characterized by acute decompensation, organ failure, and high mortality risk. Extracorporeal liver support systems have been developed to improve outcomes, but their impact on survival remains unclear. DIALIVE, a novel liver support device, shows promise in reducing ACLF progression and is associated with faster recovery compared to standard treatment. Liver transplantation is a lifesaving option for severe ACLF patients, but careful patient selection is essential for optimal outcomes. This review discusses the current perspectives on extracorporeal liver support and liver transplantation for ACLF patients.

The risk reduction persisted for at least 5 years in incidence and 10 years in mortality after a negative upper endoscopy.

Summary: This study aims to evaluate the burden of hepatitis C virus (HCV) in 2020 and forecast the burden by 2030. Using literature review, Delphi process, and mathematical modelling, it estimated the global prevalence of viraemic HCV infection to be 0.7% in 2020, corresponding to 56.8 million infections. The findings suggest that the current efforts are not on track to achieve the global elimination targets by 2030.

Novel histological scoring for predicting disease outcome in primary sclerosing cholangitis

Summary: A study found that measuring the cytokeratin 7 (K7) load in liver samples using an artificial intelligence (AI) model can serve as an efficient surrogate marker to evaluate disease progression in primary sclerosing cholangitis (PSC). The K7-AI model 2.0 was more accurate in predicting the compound endpoint compared to other serum markers and pathological evaluations, and it also provided additional variables that cannot be measured by human pathologists.

Abdominal obesity and alcohol use modify the impact of genetic risk for incident advanced liver disease in the general population

Summary: This study investigates the association between genetic variants, abdominal obesity, alcohol use and incident liver disease (ILD). Variants in genes such as PNPLA3, IFNL4, and TM6SF2 are found to be associated with ILD. Genetic risk scores (GRSs) calculated from these variants are exponentially associated with ILD, while not associated with waist-hip ratio (WHR) or alcohol use. The risk of ILD in individuals with high GRS and high WHR or alcohol use is increased by up to 90-fold.

Combined use of the ELF test and CLivD score improves prediction of liver-related outcomes in the general population

Summary: This study evaluated the predictive performance of a combination of liver fibrosis assessment, phenotype profile, and genetic risk in predicting liver-related outcomes. The combination of the chronic liver disease risk score and the enhanced liver fibrosis test significantly improved the predictive performance compared to the enhanced liver fibrosis test alone. The addition of the polygenic risk score did not significantly increase the predictive performance.
