Summary: This study aimed to define appropriate items for evaluating endoscopic activity and conventions for consistent endoscopic scoring rules in Crohn's disease (CD). The results showed that ulcers, narrowing, and healing should be included in the endoscopic scoring, while scarring and inflammatory polyps should not. The optimal method for defining ulcer depth remains uncertain. This study identified priorities for future research to develop a more representative endoscopic index for CD.

Summary: In 1992, a paper reported the melatonin content of the rat duodenum and pineal gland, and the data has been used to infer that the gut produces more melatonin than the pineal gland. However, this review presents evidence that contradicts these claims, suggesting that pinealectomy eliminates melatonin from the circulation and that the reported levels may be overestimated due to cross reactivity in immunoassays. Recent studies also show that the gut's melatonin content is much lower than originally stated.

Summary: The aim of this scoping review was to assess the use of proton-pump inhibitors (PPI) in emergency and primary care settings. However, short-term PPI administration appears to be safe with few adverse effects.

Summary: Inflammatory bowel disease (IBD) is a chronic inflammatory disease of the gastrointestinal tract, and patients with IBD may also experience extra-intestinal manifestations (EIMs). This review discusses the potential involvement of the gut microbiota in the pathogenesis of EIMs, including impaired gut barrier, microbial translocation, molecular mimicry, dysregulation of microbiota-related metabolites, and activation of immunocytes and release of pro-inflammatory cytokines. The review also summarizes microbiota-related therapies for better management of IBD-associated EIMs.

Summary: The single nucleotide polymorphism rs9557195 within the GPR183 gene is associated with increased risk of inflammatory bowel diseases (IBD) and may play a role in regulating gut inflammation. GPR183 may have a pro-inflammatory role in IBD, as demonstrated by increased surface expression on T cells with potential involvement in gut inflammation. Additionally, this SNP is also associated with higher psoriasis rates in IBD patients.

The findings showed that drug-related adverse events were common in these patients, particularly in those receiving immunomodulators and tumor necrosis factor-α antagonists. The use of multiple drugs concurrently also increased the risk of adverse events.
