Summary: Delayed cerebral ischemia is one of the main therapeutic targets after aneurysmal subarachnoid hemorrhage. This study explores the correlation between different measures of cerebral autoregulation (CA), spreading depolarizations (SD), and neurological outcome in patients undergoing surgical treatment for aneurysmal subarachnoid hemorrhage. The results show that different continuous CA indices do not correlate well with each other on a highly time-resolved basis, and the blood pressure reactivity and oxygen reactivity are the most reliable indices in identifying the risk of worse outcome in these patients. The occurrence of SD is correlated with impaired CA and may represent the pathological mechanism of delayed cerebral ischemia.

Summary: This study aimed to explore the correlation between systemic responses in the acute stage and mid-term outcomes of severe SAH patients. The results showed that systemic inflammatory responses after SAH and complications not related to operations were important risk factors for unfavorable outcomes in severe SAH patients. The predictive nomogram model based on these factors could serve as a reliable predictive tool for severe SAH patient outcomes.

Summary: Aneurysmal subarachnoid hemorrhage (aSAH) is a severe neurological disorder with high mortality and severe functional sequelae. Systemic inflammation after hemorrhagic stroke may contribute to intracranial and extracranial tissue damage. Previous studies suggest that systemic inflammatory biomarkers could be useful in predicting clinical outcomes. Anti-inflammatory treatment may hold promise in improving prognosis for patients with aSAH. This review summarizes the intricate interactions between the nervous and immune systems.

The pathophysiology of these headaches is diverse, and only a few medications have shown efficacy in alleviating the pain. An effective treatment paradigm should include evidence-based therapeutics, reduction of opioid medications if ineffective, and consideration of multi-modal pain control strategies including nerve blocks.

Summary: The study aims to evaluate whether there is an increased propensity to limit life-sustaining treatment (LST) among elderly patients in Poland from 2018-2019 compared to 2016-2017. The results show that clinicians in Poland have become more proactive in limiting LST in critically ill patients aged 80 and above during the studied period, although the prevalence of LST limitations in Poland remains low.

More research is needed to determine the reliable and conclusive evidence for the effects of milrinone.

Summary: The aim of this study was to investigate the correlation between plasma citrulline levels and plasma lipopolysaccharide (LPS) concentration, as well as the impact of sepsis on gut function in critically ill patients. The results showed no correlation between plasma citrulline concentration and plasma LPS concentration or activity. However, septic patients had significantly lower plasma citrulline levels. Additionally, abdominal sepsis was associated with higher plasma LPS activity compared to extra-abdominal sepsis.
