Summary: Children and young adults with differentiated thyroid cancer are more prone to lung metastasis, especially if they are younger, have specific tumor characteristics, and elevated post-operative thyroglobulin level. Persistent disease post-therapy is common, highlighting the importance of risk adaptive management in this population.

Summary: Risk stratification for patients with differentiated thyroid cancer (DTC) based on genetic profiling is accurate and robust, with molecular risk categories showing strong association with distant metastasis (DM) risk. Preoperative prognostication using molecular assays can help tailor treatment for patients with DTC.

Summary: Most patients with childhood DTC and DM were female and had PTC. Despite multiple RAI treatments, persistent disease was common among patients.

Summary: This study aimed to report the experience of patients with well-differentiated thyroid cancer (WDTC) who received postoperative radiation therapy (PORT). The results showed that patients who received PORT had improved locoregional recurrence-free probability, but it did not translate into improved disease-specific survival and overall survival.

Summary: This study aimed to evaluate the impact of N stage subclassification on different distant metastasis sites based on age stratification, especially for patients with papillary thyroid microcarcinoma (PTMC). The results showed that the risk of distant metastasis increased with the progress of N disease, and N1b disease had a higher risk of lung metastasis in younger patients and an increased risk of both lung and bone metastasis in older patients. N1a disease did not increase the risk of distant metastasis in PTMC patients. Combining N stage with risk stratification can be beneficial for predicting distant metastasis.

Summary: In our analysis on a cohort of N1b patients with differentiated thyroid cancers, the number of positive lymph nodes and lymph node dimension were confirmed as independent risk factors for locoregional and distant recurrence, respectively.

Summary: Patients with radioactive iodine refractory metastatic differentiated thyroid cancer have a poor prognosis. This study aimed to characterize patients with non-iodine-avid disease at diagnosis. The results showed that non-iodine-avid disease is highly prevalent in DTC patients with BVL cancers, particularly with BRAF (V600E) and TERT promoter mutations, and is associated with older age.

Summary: The study shows that in patients with hereditary MTC, the age at thyroidectomy increases with decreasing mutational risk, and primary tumors in distant metastatic patients are generally larger than nonmetastatic tumors. For sporadic MTC, lymph node metastasis is found to contribute more to distant metastasis than primary tumor size.

Summary: This study evaluated clinical strategies based on sonographic features of lymph nodes and found that detecting malignant lymph nodes through FNAC or elevated FNA-Tg had a high malignancy rate, while stratification based on sonographic features was necessary for nodes with indeterminate or benign results. A diagnostic algorithm combining FNAC, FNA-Tg, and ultrasound features was proposed for diagnosing metastatic lymph nodes of differentiated thyroid carcinomas.

Summary: Tracheal invasion in well-differentiated thyroid cancer is a negative prognostic factor, with different resection methods having varying impacts on prognosis. Patients with well-differentiated thyroid cancer generally have a good prognosis, but factors such as quality of life and surgical risks need to be considered.

Summary: This study investigated the risk factors for distant metastasis in patients with primary medullary thyroid carcinoma. The results showed that age, race, and pT stage were significantly associated with distant metastasis of medullary thyroid carcinoma. Identifying patients at high risk of distant metastasis in a timely manner is important for clinicians.

Summary: The sonographic features are closely related to the pathological results of cervical lymph node metastasis in differentiated thyroid carcinoma. The sensitivity and specificity of ultrasound for detecting lateral and central lymph nodes are high, and understanding these features can help improve the diagnostic rate of conventional ultrasonography and reduce misdiagnosis.

Summary: Patients with radioiodine-refractory differentiated thyroid cancer who have previously undergone VEGFR-targeted therapy and have no available standard of care were evaluated in this study for the tyrosine kinase inhibitor cabozantinib. Results showed that cabozantinib significantly prolonged progression-free survival, potentially offering a new treatment option for these patients.
